About the Role:

The Intake Coordinator plays a critical role in the Health Care Services team by managing the initial patient intake process at the Tyler Home Health Branch. This position ensures that all patient information is accurately collected, verified, and entered into the system to facilitate seamless care coordination. The Intake Coordinator acts as a liaison between patients, healthcare providers, and internal teams to schedule assessments and initiate care plans promptly. By maintaining organized and thorough documentation, this role supports compliance with healthcare regulations and enhances patient satisfaction. Ultimately, the Intake Coordinator contributes to the efficient delivery of home health services by streamlining the intake workflow and supporting clinical staff.


As a Intake Referral Coordinator, you will:

  • Receives and appropriately documents new referrals in a timely manner.

  • Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.

  • Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.

  • Provides effective communication to patients, staff members, other health care professionals and referral sources.

  • Responds to Agency and patient needs in a professional and creative manner.

  • Demonstrates commitment, professional growth and competency.

  • Participates in Agency-sponsored in-service training.

  • Participates in the Agency’s QAPI program.

  • Promotes the Agency philosophy and administrative policies to ensure quality of care.


Requirements

  • Minimum one year of experience in referral intake is preferred

  • Experience with third party payers, Medicare and Medicaid highly desirable

  • Medical Field background preferred, including familiarity with medical terminology

  • Excellent computer and customer service skills

  • LVN/LPN preferred

  • Valid Driver’s License

  • Valid Auto Insurance

  • HomeCare HomeBase experience preferred
