Purpose of Position: This position is to provide and/or coordinate supportive services to the residents of Callahan Flats.
Qualifications and Skills:

•Holds a bachelor’s degree in social science or equivalent experience.

•Demonstrates ability to uphold professional boundaries in relational work.
•Demonstrates interest in culturally diverse backgrounds (to include, but not limited to individuals impacted by incarceration, homelessness and substance misuse).
•Prioritizes confidentiality; exhibits time management, problem-solving, and communication skills.
•Maintains valid driver’s license, automobile insurance, and safe driving record.
Responsibilities:
•Actively engage in the provision of holistic and comprehensive case management services to residents of Callahan Flats (58); to include, but not limited to, internal services and external resource connection.
•Uphold strength-based perspective and person-first language when working with residents, teammates, and community partners.
•Drive the Callahan Flats Van to aid residents in maintaining their housing, mental health, physical health, financial and/or legal stability.
•Ensure accurate data and documentation is entered into in a timely manner, utilizing the KLF Salesforce Database System. Assist, as needed, with the reporting of qualitative and quantitative information to meet organizational and funding requirements.
•Assist team in planning, coordinating, and facilitating community engagement events for Callahan Flats residents.
•Create and distribute monthly Community Calendar for Callahan Flats residents as needed.
•Attend weekly CHAMP Coordination meetings as a Callahan Flats representative as appropriate.
•Provide task management for program interns as needed.
•Participate in all appropriate SHF program and KLF organizational staff meetings, training events, retreats, etc.
•Perform other duties as assigned

Physical Requirements:
•Ability to operate a company vehicle.
•Prolonged periods sitting at a desk and working on a computer.
•Bending and twisting.
•Must be able to lift up to 25 pounds at times.
