Job Title: Behavioral Health Counselor

Job Summary:
Provide integrated behavioral health services to address the psychological, emotional, and behavioral needs of patients served by Trillium Health.
Duties/Responsibilities:
- Complete initial assessments and determine patient diagnoses.
- Provide individual, family, and group therapy services as appropriate.
- Work collaboratively with medical providers and the interdisciplinary team to integrate behavioral health services into treatment plans.
- Develop and update treatment plans in collaboration with patients and significant others as appropriate.
- Document all treatment sessions within 72 business hours, following Trillium policy.
- Participate in clinical team meetings and supervision to support ongoing development of competencies and skills.
- Develop discharge plans and coordinate transition planning for patients completing treatment.
- Mentor or supervise interns as needed.
- Collaborate with other Trillium programs and services to support patient needs.
- Prepare regular reports on individual, group, and program outcomes as assigned.
- Provide therapeutic support and education to patients and significant others regarding mental health issues and management.
- Offer short-term motivational counseling to patients who are not yet ready or able to participate in behavioral health services.
- Demonstrate respect and support for inclusiveness and diversity.

- Maintain confidentiality and security of all protected and proprietary information.
- Participate in compliance audits, process improvement, and quality improvement plans as requested.
- Perform other duties as assigned.
Required Skills/Abilities:

Education and Experience:
- Master’s degree in counseling, social work, psychology, or a closely related field required.
- Professional licensure required (LCSW, LMHC, LMSW, LMFT); candidates practicing with a permit while working toward licensure will be considered.
- Minimum of one year of experience providing individual, family, and/or group behavioral health counseling preferred.
- Experience with trauma-informed care, co-occurring disorders, and LGBTQ+ populations preferred.
Physical Requirements:
While performing the duties of this job, the employee is regularly required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms; and talk and hear. The employee may occasionally need to stoop, bend, and lift or move up to 25 pounds. Specific vision requirements include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
