ACTIVITIES COORDINATOR

 


REPORTS TO:  ADULT DAY CENTER DIRECTOR

 

POSITION REQUIREMENTS:  Must meet one of the following qualifications: (1) Be a qualified therapeutic recreation specialist or activities professional with a certification by accredited body (2) Be a qualified occupational therapist or occupational therapy assistant (3) Have at least an associate’s degree in a discipline focusing on provision for activities for adults (4) Have a minimum of one year of full-time work experience within the last five years in an activities program in an adult care setting. Must be at least 21 years of age. 

 

SUMMARY:   Responsible for managing and coordinating the structured activities program in the Adult Day Center while maintaining personal interaction with participants and familiarity with their needs and interests.  

 

DUTIES AND RESPONSIBILITIES:

 

  1. Develops, implements, and supervises a wide variety of group and individual therapeutic activities to meet participant needs, interests, and abilities including community outings and outreach to homebound individuals or individuals not attending the Adult Day Center.
  2. Fulfills the role of Activities Coordinator within the PACE Interdisciplinary Team by actively participating in daily team meetings; collaborating and effectively communicating with other team members, participants, and caregivers; and completing all documentation required for position including but not limited to recurring assessments and individualized plans of care. 
  3. Develops and distributes monthly calendar of activities and events, monthly newsletter, and monthly lunch menus. 
  4. Assists in monitoring the general health, wellbeing, and safety of participants attending the Adult Day Center and reports concerns to appropriate discipline. 
  5. Assists in maintaining a clean, safe, welcoming, and respectful environment within the Adult Day Center. 
  6. Reports any suspected abuse, neglect, or exploitation in timely manner. 
  7. Complies with all HIPAA regulations and maintains confidentiality and privacy at all times. 
  8. Perform all agency-related services as required.  

 

KNOWLEDGE, SKILLS AND ABILITIES:

  1. Knowledge and understanding of psychosocial needs in geriatric population.
  2. Ability to work effectively both independently and within a team.
  3. Ability to effectively communicate, collaborate, prioritize, and use critical thinking skills to problem solve. 
  4. Strong interviewing, assessment, and organizing skills.
  5. Knowledge of resources and facilities used in the delivery of services to people who are elderly and/or disabled in the community. 
  6. Demonstrate interpersonal skills and human relation qualities including the ability to present a positive, caring, and inclusive image.
  7. Ability to formulate, develop, implement, and evaluate individualized plans of care.
  8. Demonstrate skills in observing, recording and reporting behaviors.
  9. Able to walk and/or stand most of the day; to work in teaching positions or strained positions most of the day; to lift, carry or handle lightweight materials (1-25 pounds or client equivalent pounds) half of the day, medium weight materials (26-60 pounds or client equivalent pounds) frequently, heavy weight materials (over 60 pounds or client equivalent pounds) occasionally; intermittent sustained muscular exertion.
