The Care Coordinator works under the direction of the Director of Nursing and is responsible for the timely completion of accurate resident assessments and interdisciplinary care plans that meet state and federal guidelines. The Care Coordinator is responsible for identifying resident acuity and helping to determine specific care needs and communicating those needs and expectations to families and responsible parties. The Care Coordinator oversees the clinical reimbursement of the facility and ensures that Medicare and Medicaid compliance is maintained. The Care Coordinator also provides oversight of the Restorative Nursing Program.
