JOB SUMMARY
Responsible for the clinical management and scheduling of services for an assigned
group of patients. Responsible for overseeing the operations and management or nurses and if applicable a home health license in the region.
REQUIREMENTS
- Education:
- A registered professional nurse licensed and currently registered in the state and in good standing with the Board of Nursing.
- Graduate of an accredited nursing program.
- Experience
- 2 years of experience in home care or home infusion therapy with management experience within the past 3 years.
- Knowledge of ACHC standards and accreditation process.
- Knowledge of regulatory requirements for home care.
- Good communication skills, detailed oriented and organized.
- Excellent computer skills to include Excel and Word.
- Excellent organizational skills.
- Able to manage multiple tasks.
- Fulltime Employment:
- 40hrs per week. Work week is Monday thru Friday
RESPONSIBILITIES
Patient Care Management
- Coordinate all patient care services for an assigned group of patients with other members of the health care team.
- Coordinate patient discharge/start of care with referral source/discharge planner.
- The Nursing Supervisor/Administrator will assure that the patient’s plan of care is implemented as written.
- The Nursing Supervisor/Administrator will assure that patient needs are reassessed by a licensed health care professional when there is a significant change in the patient’s condition, when requested by the patient’s physician and after discharge from the hospital.
- Assure that all IVIG patients are contacted 24 to 48 hours after each infusion to complete an IVIG survey regarding our services and identify immediately any care issues.
- Assure that a discharge summary at the completion of the patient’s prescribed therapy.
Clinical
- Provide clinical support to the patient, office staff, field staff and pharmacy staff.
- Based on the initial telephonic assessment and in conjunction with pharmacy, determine the appropriate administration method for prescribed therapy.
- Participate in case conferences to discuss pending referrals, current patients and clinical issues.
Patient Care Direct:
- Administers prescribed therapy to patients in the infusion suite or field according to policy.
Operational
- The Nursing Supervisor must be available to the agency at all times either in person or by telecommunication.
- Review nursing documentation in pharmacy operating system to assure accurate and thorough information.
- Track nursing visits by patient and obtain authorization for additional visits through case management.
- Participate in the company’ after hours on call system.
- Oversee the company’s Quality Assurance Program for nursing
- Audit patient medical records for assigned group of patients to assure complete and accurate documentation.
- Any other responsibility as designated by the employee’s supervisor.
- Support continuing education program for nursing staff.
- Assure compliance with ACHC standards.
Human Resources:
- Interview, hire and train new employees for the Nursing Department.
- Perform or facilitate field competency visits for new and existing nursing field employees.
- Complete annual performance evaluations on Nursing team in the office staff and the field staff.
Quality Assessment and Improvement:
- Responsible for coordinating and managing the Quality Assurance program for nursing in conjunction with the QA Chairperson.
- Responsible for reporting quarterly results to the QA Chairperson for inclusion in quarterly summary reports to the governing body.
- Responsible for reviewing all patient complaints and adverse events and evaluating trends within the office that affect patient care.
Home Health Agency Responsibilities:
- Organize and direct and oversee all functions of the home health agency.
- Assuring that the patient care documentation for services provided is accurate and timely.
- Employ a sufficient number of staff who are competent and qualified personnel in order to meet the patients’ needs.
- Ensure that the employees of the agency receive sufficient education and training necessary to perform their job responsibilities.
- Ensure that all employees of the agency are evaluated at a minimum on an annual basis.
- Ensure that information provided to the public and to the patients is accurate.
- Implement and monitor an annual operating budget for the agency to assure available funds necessary for continued patient care.
- Review all patient satisfaction surveys received.
