At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.
We are seeking a candidate who would be responsible for the overall management of a patient's case during off-business hours.
Responsbilities:
- Provides direct nursing care appropriate for a licensed RN
- Appropriately responds to after-hour calls from patients/families by educating, providing nursing care, and/or involving other team members.
- Documents activities and assessments in the clinical information system and appropriately updates team members
- Provides support to patient’s family/friends including education, expectations, and reporting changes in condition
- Supports the organization’s goals, initiatives, and value system
- Understands and is compliant to organizational policies and processes relating to regulatory, accreditation and privacy standards
- Performs other duties as required and/or requested by management
Qualifications:
- Graduate of an accredited school of nursing
- Current Texas license
- Current certification in basic life support.
- Ability to function as part of interdisciplinary team.
- Good interpersonal skills – able to communicate with team members and patients from a diverse community.
- Knowledge about hospice and experience in a hospice or hospital setting is preferred.
- Experience using a clinical information system is preferred.
- Ability to provide reliable transportation and travel to multiple work locations throughout the day.
Note: An extended orientation may be considered for an applicant not meeting experience qualifications.
