Key Responsibilities
· Outreach and engagement of community members with no primary care providers and/or uninsured underinsured within the IRMC system telephonic and in person.
· Support data collection and reporting for quality measures, community health initiatives, and grant requirements.
· Assist in planning and logistics for mobile clinic sites, community event participation, and daily operations.
· Perform general administrative tasks such as answering phones, responding to inquiries, maintaining supplies, and assisting with billing/insurance processes as needed.
· Accurately document and drop charges in the EHR related to care coordination and chronic care management services.
Mobile Clinic Tasks
· Greet and register patients in a mobile clinic setting.
· Triage patients for examination, including taking vital signs, medical histories, medication reconciliation, quality markers, and documenting in the electronic health record (EHR).
· Assist providers with examinations, procedures, point-of-care testing, specimen collection, and immunizations.
· Maintain cleanliness, organization, and stock of supplies in the mobile clinic environment.
· Follow infection control protocols and ensure safe, efficient patient flow.
· Provide patient education on care plans, medications, chronic disease management, and preventive health.
· Schedule follow-up appointments, specialty referrals, and diagnostic testing as directed by providers.
· Serve as liaison between patients, providers, and external agencies for continuity of care.
· Monitor and coordinate patient outreach and follow-up for test results, chronic care, and preventive screenings.
· Manage patient records and ensure accurate documentation in the EMR.
Qualifications
· Minium completion of accredited Medical Assistant program, LPN or RN.
· Current Medical Assistant certification (CMA, RMA, or equivalent).
· CPR/BLS certification required (or obtained within 90 days of hire).
· Previous experience in primary care, urgent care, or community health setting strongly preferred.
· Strong organizational, communication, and multitasking skills.
· Valid drivers license
Working Conditions
· Comfort working in a mobile, community-based environment and serving diverse populations.
· Ability to work independently and as part of a multidisciplinary team.
· Proficiency in electronic health records and basic computer applications.
· Ability to lift and move medical equipment and supplies (up to 30 lbs).
