Case Manager Position
Position Summary:
Wiley is seeking a dynamic and compassionate Case Manager to support our subacute rehabilitation program and represent our organization within the broader healthcare and senior living community. This highly visible, field-based position plays a key role in both outreach and patient experience.
The Case Manager will establish and maintain strong referral relationships with hospitals, senior living communities, and healthcare professionals. They will also serve as the main point of contact for patients and families transitioning to Wiley for subacute rehab, ensuring a smooth and supportive experience from admission through discharge.
Key Responsibilities:
- Develop and maintain relationships with hospital discharge planners, case managers, and social workers to identify and coordinate appropriate referrals for subacute rehab at Wiley
- Regularly visit hospitals, assisted living communities, independent living communities, physician practices, and senior centers to promote Wiley’s full range of services and generate referrals
- Actively participate in community events and networking opportunities to expand awareness of Wiley’s offerings
- Coordinate the admission process with internal and external partners to ensure timely and seamless transitions
- Personally welcome each new subacute rehab patient upon arrival and serve as the consistent point of contact for the patient and/or family throughout the stay
- Collaborate with admissions, nursing, therapy, and social work teams to support a coordinated and patient-centered care experience
- Track and report outreach activities, referral trends, and patient feedback to support ongoing quality improvement
Qualifications
- Current NJ Nursing License
- Background in healthcare marketing, admissions, case management, or discharge planning strongly preferred
- Strong understanding of the post-acute care continuum and hospital referral processes
- Excellent communication, interpersonal, and customer service skills
- Warm, empathetic demeanor with the ability to support patients and families during transitions of care
- Self-motivated with the ability to work independently and manage a field-based schedule
- Valid driver’s license and reliable transportation required
