RATE OF PAY/SALARY: $21 per hour

Description of Duties:
- Data Entry into Electronic Medical Records called eClinicalWorks (eCW)
- Navigate the Patient Navigation Proxy/Cancer Services Proxy in eCW
- Assist with care coordination related to cervical cancer screenings, colonoscopies, Fit Kits, and other related preventative screenings such as annual visits.
- Schedule medical appointments with all specialties and services within Cornerstone and with external providers.
- Conducts outreach to patients who have missed preventative screening appointments.
- Collaborates with all the patients’ providers within the Cornerstone Family Healthcare system and externally to ensure all pertinent patient information and referrals are received in a timely fashion for the completion of medical care.
- Monitor and track patient progress in completing screenings successfully.
- Report findings on a regular and timely basis in eCW for Cornerstone Providers to access information needed for medical care.
- Assist patients with obstacles such as medical transportation trips and translation services.
- Referrals to community services such as behavioral health services, housing, WIC, food pantries, applying for medical insurance(s), Department of Social Services, et. al.
- Maintain community relationships via networking and collaboration of services.
- Educates patients on the importance of preventative screenings and positive health outcomes.
- Educate patients on how to navigate medical care system such as scheduling appointments, getting medication refills, scheduling transportation trips, et. al.
- Assist with filling Gaps in Care for quality improvement.
- Assist Quality Improvement with eCW campaigns for health-related services.
- Comply with HIPAA laws and Cornerstone Family Healthcare confidentiality policies and procedures.
- Attend and participate in department, All Staff and other required meetings and trainings.
- Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other related duties as assigned.
Requirements
- Medical Assistance experience preferred or at least 1 year of relevant medical experience
- Bilingual (English/Spanish)
- Computer skills and EHR skills highly preferred
- Ability to maintain a non-judgmental disposition within a diverse population
- Knowledge of community resources a plus
- Desire and ability to work closely with low-income population
