About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY/OVERVIEW: The Behavioral Health Clinician (AMFT/LMFT, ASW/LCSW, APCC/LPCC) provides therapy to clients across the lifespan with mild-moderate mental health conditions. Services are provided in the clinic, and some services may be via telehealth. The Clinician is a part of a team and offers ideas, feedback and solutions on a range of topics and supports their fellow team members to do their best work. The Clinician demonstrates cultural humility and competence providing services for those who experience a wide range of symptoms, conditions, and psychosocial factors. The Clinician actively collaborates with other clinical, case manager, peer, and supervisory staff by using a strength-based and client focused approach where learning and professional curiosity is key. The Clinician understands the role that stigma, discrimination, and systemic factors can play in a client’s ability to experience safety, attain wellness, and thrive. | |||

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1. | Responsible for responding to crisis, triaging client needs, conducting comprehensive biopsychosocial assessments, diagnosing, treatment planning, providing therapy, and coordinating care. |
2. | Understand and comply with Medi-Cal service delivery and documentation requirements, promptly responds and corrects all documentation errors once identified. |
3. | Complete clinical documentation within 48 hours of service delivery in accordance with working hours and agency expectations. |
4. | Correctly utilize and interpret ICD10 and DSM5 codes and criteria. |
5. | Effectively and skillfully deliver any combination of service options: individual, couples, family, and group therapy to persons across the lifespan. Services may occur in clinics and via telehealth based on the needs of the program and client. |
6. | With client permission, collaborate directly with primary care providers, case managers, peer advocates, supervisor, community partners, and County Mental Health Department when indicated. |
7. | Demonstrate cultural humility and respect when working with clients, peers, and community partners. Takes professional responsibility for closing knowledge and skill gaps, actively seeks consultation and feedback. |
8. | Monitor charts to ensure the timely placement of all correspondence and associated paperwork. |
9. | Complete all screening and data collection tools, tracks caseload. Participate in quality assurance and improvement efforts, including peer chart review. |
10. | Manage complex caseload, direct service, and administrative duties in a fast-paced setting with attention to detail. |
11. | Create client-driven treatment goals and plans, all plans to be kept current. Clinical interventions are aligned with treatment plan goals and evidenced in the documentation. |
12. | Client progress is measured utilizing a combination of some/all of the following: diagnosis specific screening tools, client / caregiver reports, clinical observations, and completion of treatment plan goals. |
13. | Responsible for detecting emotional, psychological, and physical risk factors and danger signals and co-creates safety plans with clients/caregivers when needed. |
14. | Create a supportive discharge process and plan, refer as needed. |
15. | Responsible for remaining within all legal/ethical standards, including confidentiality and HIPAA, mandated reporting, Tarasoff, Fraud, Waste, and Abuse. |
16. | Responsible for understanding, explaining, and enforcing all rules and regulations associated with clinic policy. |
17. | Compile and report on outcome data as required. |
18. | Respond to all emails, voicemails, client communications, departmental/agency communications promptly. Maintains accurate schedule and proactively communicates changes. |
19. | Attend clinical supervision/consultation, weekly staff meetings, meetings with community partners, training as arranged/required. |
20. | Maintains current license/registration in good standing, submits all CEU and training certifications to HR. |
21. | Responsible for maintaining the standards as listed in the CommuniCare+OLE Health Centers Policies and Procedures, and any other standards which may apply to this position.. |
22. | Performs other duties as assigned. |
