Behavioral Health-Patient Navigator

Pay Rate-$24.15-$38.65 (Compensation will be determined based on qualifications and experience)
About the Role:
The Behavioral Health Patient Navigator assesses patient needs, creates service plans, connects them to community resources, and collaborates with HICHC departments and partners to address social determinants of health.
Minimum Qualifications:
- Required-Bachelor’s degree in relevant human services field plus at least one-year experience required; OR any equivalent combination of experience, training and/or education.
- Preferred-Staff are expected to be Tobacco Treatment Specialist (TTS) eligible (meaning a non-nicotine user for at least 6 months) or hold TTS certification (
- Preferred-Previous experience working in a team environment, clinical experience working with diverse clinical populations and knowledge of the Hawaiʻi Island community
- Preferred-Eligible for future licensure in Social Work, Counseling, or other related field
Responsibilities:
- Participates as directed in the screening of new service requests within the clinic by means of agency approved procedures that ensures all individuals are adequately and appropriately served according to their individual needs and in accordance with existing HICHC policies and procedures
- Works directly with patients to reduce barriers and social determinants of health (SDOH) issues to improve health care access and patient quality of life.
- Educates patients on social determinants of health and assists with navigating various systems

Able to work independently, manage multiple tasks in a fast-paced setting, and remain emotionally stable and mature. Skilled in understanding client needs, evaluating problems, and developing effective treatment approaches.
