QUALIFICATIONS:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. The Licensed Practical Nurse will perform liaison duties for the medical department, maintain medical supplies, coordinate patient assistance process, obtain prior authorizations for medications and other assigned duties to assist physicians in meeting consumer needs. The LPN will also provide liaison services with psychiatrist on behalf of individuals with BH/ID diagnosis, attend all psychiatric visits, and follow-up with all psychiatric orders on behalf of assigned individual’s needs.
QUALIFICATIONS:
- Successful completion of LPN program.
- At least 1 year experience in the provision of nursing care.
- Active PA LPN license in good standing.
- Valid Driver's License and personal vehicle.
- Ability to use an electronic health record, e-prescribing and e-lab software.
- Good organizational skills and attention to detail.
PHYSICAL DEMANDS:
Minimal in nature for this position. Physical ability to perform CPR.
SPECIFIC DUTIES AS A LICENSED PRACTICAL NURSE:
- Work in conjunction with PATH’s medical department to provide ancillary services as requested by psychiatrist.
- Call medication into pharmacy, or enter order through e-prescription software, as directed by physicians
- Call medical specialists, residential programs, etc. as directed by physicians
- Maintain medical supplies and medications, ensuring that expired medications are disposed per PATH policy.
- Meet with pharmaceutical representative as assigned
- Coordinate and monitor patient assistance program as assigned
- Complete and obtain prior authorization for medications as assigned by physicians
- Distribute labeled medications, including by injection, as assigned by physicians
- Evaluate and assess the needs of individuals on caseload who are diagnosed with behavioral health and intellectual disabilities
- Coordinate efforts directed toward meeting the needs of individuals on caseload
