Essential Functions

1. Ensures the MDS process runs effectively: Completes care plans with the Interdisciplinary team on a timely basis. Performs assessments of all residents to generate the MDS and monitors accuracy of MDS and Nurse assessments prior to transmission. Completes the submission of the electronic record within 30 days of MDS completion. Ensures that assessments are completed and signed in a timely fashion and discussion any concerns regarding compliance of team members to DON or Administrator. Attends weekly resident update meeting to discuss MDS data with the team. Conducts audits on a regular basis. Reviews each resident’s care plan and documents accordingly for triggers not care-planned for initially. Initiates MDS care plans for residents in their assessment period to ensure documentation is obtained and calculates triggers and use resident assessment protocols for initiation of care plan.

2. Manages the MDS process for the skilled nursing unit: Maintains a working knowledge of MDS requirements and RAI process. Monitors interdisciplinary team members to ensure that documentation is timely and complete. Maintains current resident assessment schedule and identifies staff responsible for completion of assigned sections of assessment. Becomes proficient in use of AOD systems and communicates system concerns with the SOD vender to keep process active. Identifies significant change in status of residents and coordinates weekly and 48-hour interdisciplinary care conferences. Understands the relationship between MDS, Resident Assessment Protocols and Medicare SNF Prospective Payment System and how they interrelate. Becomes Proficient in ICD10 coding to ensure accuracy and that all diagnoses are coded for reimbursement. Maintains the RAI manual with current updates and completes monthly reports with analysis on schedule. Reviews the CASPER Report and provide detailed explanation into the trends and triggers on the report. Reports findings to the DON and to QAPI Committee. Completes audits of the daily charting to ensure the information being reported in the MDS is accurate (providing explanations as necessary).

3. Coordinates the Restorative Nursing Program by working with Therapy in communicating the resident’s restorative program to the Certified Nursing Assistants (CNA), making sure the Care Plan is updated to reflect the restorative goals, auditing the CNA documentation weekly for compliance and notifying the DON of non-compliance.

4. Leads various Quality Improvement committees and workgroups such as the MDS/Documentation Team and Infection Control Team. Attends and participates in quarterly QAPI meetings, communicating thoughts and directives clearly and effectively to the team. Participate in team problem solving. Practices flexibility with scheduled working hours as needed to meet department needs. Conducts and attends meetings as designated.

5. Promotes education of the MDS process and documentation: Trains appropriate interdisciplinary staff in the completion of the MDS process and AOD program. Instruct staff in the use of assessment tools and provide periodic educational programs to nursing staff on assessment and MDS related issues.

6. Review resident charts on a routine basis to verify proper documentation, identify discrepancies, and ensure compliance with state, federal, and facility-specific regulations.

7. Provides leadership to the Health Services Staff: Participates as a member of the nursing leadership team, leading special projects as assigned by the DON and or Administrator.

Education and Experience

Graduate of an approved school of Registered Professional Nursing and current licensure by the Commonwealth of Pennsylvania as a Registered Professional Nurse. Must maintain an active CPR Certification. Minimum of two years’ experience in Long Term Care Setting as an RNAC. Excellent computer skills and strong clinical assessment and leadership skills are required. Effective organizational, problem solving, writing and interpersonal skills required. Must be detail-oriented.
