About the Department
This position coordinates services for children and their families enrolled in the Children’s Long-Term Support (CLTS) and Children’s Community Options Program (CCOP). The focus of this position is to coordinate services, develop individual service plans and to work with all involved to help empower families with the skills and resources they need to address their child’s developmental, physical or severe emotional disability.

Position Duties
This job description reflects management's assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned.
Responsible for managing a caseload, coordinating and facilitating the development of Individual Service Plans that meet the needs of families and maintain measures of progress in meeting outcomes. This includes on-going monitoring and case management practice as well as the ability to facilitate and participate in meetings with a variety of people in an array of settings, including in the participant homes throughout Walworth County.
Document all case management activities and complete paperwork as required by the agency and all pertinent State and Federal guidelines.
Understand the limitations and requirements of program budgets, funding, costs, and choices; ensuring resources are effective, efficient, and have value to the participant. Reviews invoices, service codes and create authorizations for payment.
Collaborate with programs such as Comprehensive Community Services as well as other individuals across the Health & Human Services department to ensure proper and effective coordination of care.
Demonstrate a commitment to county safety and risk management efforts.
Minimum Qualifications

Bachelor’s Degree in Social Work or Human Services related field with a minimum of one year of experience working with children, families, or individuals with disabilities. Ability to communicate in Spanish preferred. Proven experience in the ability to read, write and interpret legal documentation as required in Wisconsin Statutes and Standards. Knowledge of Trauma Informed Care principles is essential.
Certificates, Licenses, Registrations
Certification in the Children’s Long-Term Support Functional Screen or the ability to obtain certification within one month of hire.
Valid driver’s license.
Proof of minimum automobile liability insurance coverage.

Proven ability to effectively communicate specialized information and/or recommendations to others regarding an area of expertise.
