Self-Directed Services Representative bei Lakes Region Community Services Council

Description

Lakes Region Community Services (LRCS) is recruiting for a full-time (37.5 hours per week) Self-Directed Services (SDS) Representative to provide support and assistance to individuals and families in coordinating and managing their SDS services. The successful candidate shall possess the following:

1. A mix of business orientation with programs and services; an ability to comply with the rules & regulations with individuality.  
2. Listens to, and help draw on, their hopes and dreams. Encourage individuals in community participation and intentional relationship building. Support transition from the Local Education Agency to the adult system, as needed.      
3. Maintains current information on future planning. Participates in IEP meetings as applicable and requested. Facilitates team meetings and develops Service Agreements for LRCS services
4. Empowerment – The ability to support and encourage individuals to make their own decisions through the teaching and learning process.
5. The ability to achieve a balance between respecting family values while working within the boundaries of rules and health & safety.

Requirements

 Qualified applicants will have prior casework or related experience. Bachelor’s degree in education, social work, or related field required. Possesses effective interpersonal, communication, and organization/time management and computer skills. Must be self-directed, able to think quickly and work independently. Knowledge of Belknap and Southern Grafton County communities for available resources is preferred.
