Care Coordinator Hiring Event bei Center for Human Development (CHD)

Center for Human Development, (CHD), will be hosting a hiring event on Wednesday, September 24, 2025, from 10am-1pm. We are seeking Care Coordinators for our Innovation Care Partners Program. Care coordinators work with enrollees from 3 years of age up to 65 years of age.  The Care Coordinators role is to work with a large caseload of enrollees providing evidence-based care coordinator and management. ICP Care Coordinators ensure that more people in our communities can benefit from all exception health services. You will collaborate with the participants care team in support of health and wellness education and advocacy. Providing community resource awareness and support. You will conduct home visits, accompany / transport enrollees to health appointments and support navigation of the health care system.

Locations include:  West Springfield, Northampton and Pittsfield, MA. With outreach in surrounding communities. 

Requirements:

• Candidates should have a valid driver's license,
• Must have a vehicle available for work purposes that is registered and insured.
• Bilingual (English/Spanish) encouraged to apply.
• Must hold High School diploma or equivalent. Bachelor’s degree level applicants are encouraged to apply.

Pay rate starts at $21.00/hourly. This is a full-time opportunity.
