Overview:

The Case Manager is responsible for the coordination of services for clients.

Min:USD $54,750.00/Yr.Max:USD $65,000.00/Yr.Responsibilities:

Other duties may be assigned as necessary.
• Provides case management services to clients
• Participates in development of assessment, service delivery, discharge, and aftercare planning
• Maintain accurate, timely, and complete documentation according to licensing, accreditation and agency standards.
• Attends trainings and meetings as required.
• Provide six months of aftercare case management for clients.
• Participates in case planning conferences, Professional Consulting Team, School meetings, court hearings, staffing, etc., as appropriate.
• Maintains regular contact with child and monitors psychological, mental, emotional, physical, and environmental well-being.
• Provides crisis intervention as needed to prevent escalation of high-risk behavior, relapse, hospitalization, placement disruption, and other negative outcomes.
• Inputs and updates child and family information into the Arrow client information system.
• Review client files on a quarterly basis to access service plan implementation, progress towards desired outcomes, and future appropriateness of service plan.
• May be required to drive ACFM vehicles, be insurable with our auto insurance provider, and complete Arrow’s vehicle training
• Perform all duties within contract, state and federal regulations, Arrow policy, and accreditation standards.

Qualifications:

Bachelor’s Degree in Social Work, Social Services or a related field and at least one (1) year experience working with children and families. Master’s Degree preferred.
