About Us:

Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network works together to create opportunities for Ohioans to receive needed home and community services and support and to age successfully in their homes and communities.

Position Summary:

The Transitional Care Home Coach plays a crucial role within the member-centric Transitional Care Team by serving as the initial point of contact between members and the Western Reserve Area Agency on Aging. This role is vital in promoting the well-being and independence goals of members and fostering collaboration and coordination of services with managed care organizations. A successful Home Coach seamlessly integrates into the TC Team to offer member-centered coordination of services.

 

Job Duties:

 

  • Provide orientation and explain program details to participants, including information on medications, the scope of care and transitions, the coaching process, program expectations, and available agency resources.
  • Visit program participants at their homes or hospitals, using in-person meetings, videoconferences, or phone calls based on program requirements.
  • Perform and document prescreening assessments, chart histories, and initial intake evaluations for each participant.
  • Coach participants on achieving personal goals, managing health conditions, adhering to medication schedules, following up on medical appointments, and practicing self-advocacy.
  • Educate participants and their caregivers on agency resources and provide necessary support.
  • Advocate for participants and/or their caregivers/families, ensuring access to appropriate services aligned with their insurance and agency resources.
  • Utilize clinical expertise and critical thinking skills to carry out program activities in accordance with education, experience, and professional practice standards.
  • Report any cases of abuse, neglect, or exploitation of older adults, as required by Ohio law and program guidelines.
  • Follow the professional code of ethics set by the Ohio Board of Nursing and WRAAA.
  • Consistently demonstrate effective customer service with both internal and external stakeholders to achieve departmental goals, program metrics, and the agency's mission.
  • Engage in training, meetings, in-services, and continuing education opportunities provided by the agency, managed care partners, or as directed by the employee, and respond to requests from supervisors to support departmental objectives.
  • Be willing and able to travel to community and participant locations within the service areas (Cuyahoga, Lorain, Geauga, Lake, and Medina counties).
  • Work collaboratively with external agencies, internal departments, and partner AAAs to ensure uninterrupted care for program participants.
  • Carry out additional tasks as required by TC Management to fulfill agency and departmental goals.
  • This position primarily involves significant agency-related responsibilities that require discretion and independent judgment.
  • All staff are encouraged to propose quality improvement ideas and actively engage in initiatives aimed at enhancing the quality of services.
  • Support and engage in additional programs as necessary.

 


Requirements:

 

  • Possess a current, unencumbered, and unrestricted LPN, license in the state of Ohio.
  • Hold a minimum of one year of clinical experience post-licensure.
