Under the general supervision of the Functional Eligibility (FE) Manager, the Functional Eligibility (FE) Supervisor provides daily oversight to the team of Functional Eligibility Specialists (FES). This role is critical in ensuring the optimal performance of Lakeland Care, Inc.’s (LCI’s) Long-Term Care Functional Screen (LTCFS) program. The supervisor is responsible for meeting LCI’s quality standards, adhering to contractual requirements, and supporting the eligibility process for members.
Essential Competencies:
- Monitor performance standards for Functional Eligibility Specialists, establishing accountability measures and providing feedback when performance standards are not met.
- Support development and maintenance of systems and polices essential for conducting functional screens
- Implement improvement strategies to support the screening process.
- Ensure compliance to organizational policies, procedures, best practices, and State and Federal regulations.

- Maintain the State of Wisconsin Long Term Care Functional Screen certification.
- Proactively identify current or potential barriers within the LTCFS program.
- Provide solution focused ideas that address the root causes and prevent future barriers. Support development and implementation of solutions that address root causes and prevent future issues.
- Analyze and apply data in collaboration with the Functional Screen Quality Specialist related to the effectiveness and quality of LCI’s functional eligibility efforts.
- Review validity of rescreen requests and prioritize as needed.
- Assist with identifying, developing, and implementing training opportunities to strengthen understanding and application of the LTCFS.

Requirements
Qualifications:
- Bachelor of Arts or Science degree, preferably in a health or human services related field, or have a license to practice as a registered nurse in Wisconsin pursuant to Wis. Stat. § 441.06.
- A minimum of five (5) years professional experience in a long-term care organization or community-based setting preferred; at least one year of professional work experience with one or more of the target groups served by Family Care is required.
- A Minimum of two (2) years of leadership/supervisory experience is preferred.
- Strong knowledge of the Wisconsin Long Term Care Functional Screen.
- Current State of Wisconsin Long Term Care Functional Screen certification; or will pass the online LTCFS Training Course within 30 days of hire.
- Completion of all mandatory certification courses, exams, refresher courses, and continuing skills testing as required by the Department of Health Services (DHS) to maintain LTCFS Certification.
- Ability to travel throughout all regions served by the organization and throughout the state of Wisconsin.
- Current driver’s license, acceptable driving record and proof of adequate insurance required.
