Candidate must be a clinically competent registered nurse. The CCM nurse works with patients to educate them on chronic disease management in the patient’s home. This position will also have a special focus on diabetes education. Candidate counsels the patient and family in meeting nursing and related needs, participates in in-service programs, supervises and teaches other nursing personnel. Graduate of an accredited school of nursing currently licensed in the State of Ohio.
Responsibilities
- Provides direct patient care
- Provides education to patients/family on chronic disease processes
- Provides direct consultation using in-depth clinical knowledge and skills in area of Diabetes to groups and individual patients with Type 1, Type 2, Gestational Diabetes and Pre Diabetes patient
- Receives and assesses referrals from referring providers.
- Consults with multidisciplinary team as appropriate to develop, implement and evaluate care for patients and families.
- Documents patient care activities in patient medical records
- Complete documentation accurately and timely.
- Coordinate patient care with appropriate disciplines.
- Physician consultation for changes in patients' condition.
Requirements
- Registered nurse with two years of experience in delivering nursing care
- Analytical abilities and sound nursing knowledge.
- Effective communication skills.
- Knowledge of policies and procedures.
- CPR certification.
- Reliable transportation, current driver's license and automobile insurance.
- LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
