We are seeking a Registered Nurse/Diabetic Educator to join our team and lead initiatives to improve quality outcomes, specifically focused on A1C management and diabetes care.

Position Summary
The RN/Diabetic Educator will play a key role in supporting patients with diabetes by providing education, care coordination, and disease management strategies. This role will focus on improving A1C and Blood Pressure outcomes, reducing complications, and enhancing overall patient engagement and self-management. The ideal candidate is passionate about patient education, chronic disease management, and quality improvement in healthcare.

Key Responsibilities

  • Provide individualized diabetes self-management education and support (DSME/S) to patients and families.
  • Collaborate with providers, care managers, and interdisciplinary teams to identify patients with uncontrolled diabetes and/or hypertensive patients.
  • Develop and implement personalized care plans aimed at improving glycemic and blood pressure control.
  • Educate patients on lifestyle modifications including nutrition, physical activity, and medication adherence.
  • Monitor patient progress, track A1C/BP outcomes, and report results for quality improvement initiatives.
  • Participate in community outreach and health education programs to promote diabetes and hypertensive awareness.
  • Maintain accurate documentation in the electronic health record (EHR).
  • Support clinical quality programs such as HEDIS, P4P, and NCQA requirements.
Requirements

Qualifications

Qualifications include:

  • Education: Graduate of an accredited Registered Nurse (RN) program
  • Current Basic Life Support (BLS) certification for Healthcare Providers sponsored by the American Heart Association.
  • Certified Diabetes Care and Education Specialist (CDCES) preferred (or willingness to obtain within 1 year).

Skills/Experience:

  • Minimum 2 years of nursing experience, preferably in chronic disease management or primary care.
  • Strong knowledge of diabetes care, including medication management, insulin therapy, and lifestyle interventions.
  • Excellent communication, motivational interviewing, and patient education skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Experience with EHR systems and quality reporting a plus.
  • Current state driver’s license      
  • Unlimited access to a reliable car
  • Preferred English and Spanish speaking

Working conditions

Normal clinical office environment. Occasional overtime may be required and/or hours may be shortened as business needs dictate.

 
 

Physical requirements

Requires both sitting and standing for long periods at a time. Due to the nature of nursing there are periods of time where heavy lifting may be necessary. Occasionally manual labor may be required. The physical requirements of this position may evolve as duties change.
