Oklahoma City Indian Clinic (OKCIC) is a non profit corporation that provides culturally sensitive health care to the American Indian population. OKCIC services not only include basic medical care, but also dental, optometry, behavioral health services, fitness, nutrition and other family programs.

Oklahoma City Indian Clinic is looking for a passionate Registered Nurse Case Manager eager to join our team and provide professional case management principles in a patient centered medical home setting. Ideal candidates will be energetic, highly motivated, flexible team players, and readily adapting to a quick paced environment. This position is responsible for reporting and notifying patients of pap smear results and facilitating transfer of obstetric care.
Some essential duties to include:
- Follows the Nursing and Case Management process including assessment, planning, implementation, coordination and interaction through the referral process.
- Coordinates and manages communication with patients, family and health care team to access health care services not available at OKCIC by utilizing referral resources (i.e. Indian Health Services, Medicare, Medicaid and other community resources).
- Ensures that services support quality patient communication, education, financial and tribal components. Records in EHR and RPMS accurately and timely. Responsible for processing referrals accurately and in a timely manner.
- Communicates with the primary care provider/health care team member regarding patient referrals to assess outcome of referral for further treatment of the patient.
- Provides patient education regarding the purpose of the treatment/referral and the intended outcomes. Documents patient progress through the continuum of care.
- Provides nursing support for physicians, nurse practitioners and physician assistants.
- Assesses patient needs to coordinate and manage patient care within the clinic, including triage and scheduling patient’s appointments.
- Assesses patients, obtains a physical assessment and medical history; assesses patient needs and communicates needs to primary care providers to coordinate care.
- Responsible for the overall management of team approach nursing and coordination with other disciplines to provide quality care. Performs specialized skills as per clinic policies and procedures.
- Participates in assigned duties as designated by the VP of Nursing to include, but not limited to participation on designated committees, inspection of equipment and refrigerator logs.
- Maintains professional knowledge and proficiency through continuing education, in-services and workshops.

Qualifications
- Due to the safety sensitive nature of this position, medical marijuana license holders who test positive for marijuana will not be qualified for this position.
- Must align with OKCIC vision, mission and core values.
- Associates in Nursing Required, BSN preferred.
- Must possess a current Oklahoma Nursing License in good standing with the OK State Board of Nursing.
- Current CPR is required (or must be obtained within 6 months of hire).
Experience Preferred:
- Knowledge of Resource and Patient Management System (RPMS) and Electronic Health Record (EHR)
