Job Description: 

Summary:

This position is responsible for internal sales and relationship building activities within the EverTrue Life Plan Communities.  This position is responsible for establishing and maintaining effective working relationships with referral sources, customers, and EverTrue team members.   This position is also responsible for coordinating the necessary services those prospective and/or current clients will need and supervising the staff involved in direct client care.  The Private Duty Liaison upholds the philosophy, vision, and core values of the organization and ensures the Christian Mission of EverTrue is realized so that “Older Adults Live Life to the Fullest”.

Essential Duties and Responsibilities:

1. Establishes and maintains effective working relationships with referral sources, EverTrue life plan communities, and other key stakeholders

2. Effectively markets and sells to EverTrue life plan communities to generate appropriate referrals and program enrollments

3. Identifies and pursues new opportunities to increase appropriate referrals and program enrollments

4. Promotes EverTrue as the private duty service provider of choice in the St. Louis metro area

5. Coordinates, schedules, and conducts private duty programs to educate residents, families, and staff about programs and services that are available through EverTrue

6. Tracks appropriate information in the database throughout the marketing and sales process

7. Tracks needs of patients on an as-needed basis to assist with customer service items and supervisory visits of Private Duty staff

8. Establishes a positive image of EverTrue in the community

9. Maintains a high standard of excellence by promoting a positive, productive, and compassionate customer experience, always striving to exceed customer expectations and treating patients, residents, guests, clients, families, physicians, visitors, vendors, and coworkers with dignity and respect

10. Promotes an atmosphere of teamwork, emphasizing a group effort in which the contribution of every employee is essential

11. Meets with the Director of Private Duty to discuss goals and expectation on an as-needed basis

12. Completes initial assessment of client to determine services and completes necessary paperwork and enters information in computer in a timely manner. Including, obtaining a signed contract for each client that clearly defines services and rates, and submits to office in a timely manner.  Complete supervisory visits on all clients in accordance to EverTrue policies and procedures.

13. Updates client care plan when services or level of care changes, informs office and direct care staff of changes immediately and submits to office in a timely manner

14. Updates client information in the software system in a timely manner

15. Prepares equipment and materials for treatments observing aseptic technique as required

16. Coordinates client medication management, sets up meds weekly, reorders meds, and communicates with family and physician as necessary, and documents all changes

17. Instructs, supervises and evaluates the services to the client in accordance with EverTrue policy

18. Follows up on requests for services, equipment and other necessary items that a prospective or current client may need

Qualifications, Knowledge, Skills & Abilities:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  • Bachelor’s degree from an accredited college or university with emphasis in nursing, social work or business/marketing; and/or commensurate experience in marketing/sales of home and community-based services
  • Knowledge of Medicare, Medicaid, and managed care home health and hospice services
  • Excellent speaking, writing, and sales skills
  • Ability to travel daily within assigned territory
  • Self-motivation to work independently and as a team member
  • Advanced knowledge of computers and the Internet
  • Database experience 

In addition to the above minimum qualifications, this position must also possess:

  • Demonstrated skills and ability to effectively present information to management and public groups
  • Ability to interact effectively with referral sources, EverTrue team members, residents, clients, families, visitors, and the general public
  • Ability to implement and interpret programs, goals, objectives, policies, and procedures.
  • This position will operate fairly independently and will require frequent local travel to Metro St. Louis EverTrue CCRCs to generate appropriate referrals and program enrollments with emphasis on hospice, private duty, and home health.
  • The individual must be comfortable in a clinical environment and have a general knowledge of medical terminology.

A well-lighted and ventilated working area that has temperature control system (air conditioning and heat); The work will have some short notice requests, timelines for completion of tasks or reports and some major projects extending over months; The employee will experience frequent travel by car with occasional overnight stays. Some exposure to outside weather conditions in the course of travel; Minimal exposure to infectious diseases and blood borne pathogens; Minimal exposure to chemicals and hazardous waste.

EverTrue Pay & Benefits

Pay grade: E ($62,300/year to $99,900/year)
