Your role and responsibilities

In this role, you’ll work directly with people receiving services to review their care and support plans, ensuring they meet individual needs and promote independence and wellbeing. You’ll be a key part of our mission to deliver high-quality, personalised outcomes that are economically sustainable.

On a typical day, you might:

  • Assess eligibility under the Care Act 2014
  • Arrange care packages tailored to individual needs
  • Evaluate decision-making capacity under the Mental Capacity Act 2005
  • Apply safeguarding policies and procedures
  • Review existing services to ensure quality and compliance
  • Complete checklists under the Continuing Health Care Framework
  • Conduct phone assessments, home visits, and attend professional meetings

This role involves lone working in the community, and you’ll be supported through a comprehensive induction, shadowing opportunities, and ongoing training.
