Summary
The Advanced Practice Provider (APP) works independently, and in collaboration with the supervising physician to deliver excellent primary and chronic care management to a geriatric and palliative care population within the community primarily through a telehealth platform. The APP provides services within the APP scope of practice including the management of symptoms or disease processes related to acute and chronic conditions, assisting with communication and coordination of care, timely disease-specific education, as well as developing treatment plans that align with the patient’s goals throughout the disease process. The APP would provide recommendations for and/or orders including pharmaceutical interventions/medications, laboratory, and diagnostic services as needed, and will work closely with the patient and caregivers to maintain close engagement for optimal management. Assessment of needs may include utilization of additional IDT resources including social work or chaplaincy as needed. Due to the variety of variables that may impact navigation of care for this population, the APP must be flexible, demonstrate initiative, organization, and excellent professional communication with ability to build rapport rapidly with patients and their caregivers for high patient and caregiver satisfaction.
As a newer service line of care, the APP will exhibit positivity, autonomy, problem solving capabilities, and ambition to build and grow the community-based service line through excellent evidence based care that is holistic in nature and provides optimal outcomes.
Essential Duties and Responsibilities
- Under the direction of the designated supervising physician, diagnoses medical conditions, plans and implements interventions, and evaluates patient response.
- Assembles and documents complete patient medical database including health history and physical examination.
- Documents assessments, orders, interventions, results and planning in the medical record.
- Communicates findings with supervising physician, external medical team involvement, and IDT (interdisciplinary team) members as appropriate.
- Provides information, education, and support to patients and families.
- Facilitates communication between patients, family members, and the IDT.
- Fosters teamwork and serves as a resource within the IDT.
- Promotes and practices economical and effective primary geriatric - palliative care.
- Assesses eligibility for hospice services
- Exhibits behaviors that reflect commitment to the philosophy and vision of Alive
- Participates in team meetings as required
- Potential to serve as a mentor and preceptor to nursing students, medical students, medical residents and fellows
- Assists in orienting new providers to the Alive team as needed
- Maintains all State and National certification (discipline specific), including active and unrestricted DEA, and demonstrates continued competency in the management of patient care
- Participates in required continuing education to maintain credentialing and compliance for state licensure and national certification;. Remains current on research findings and integrates into practice.

Requirements
EDUCATION and/or EXPERIENCE
Graduation from an accredited APP degree program (ex: NP, CNS, PA); Masters level degree preferred. Experience in geriatrics, hospice or palliative care preferred. Experience in community based chronic disease management preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Current/Active and unrestricted Tennessee licensure, and maintenance of national certification in associated discipline (ex: NP/CNS/PA) along with ability to maintain unrestricted DEA for prescribing practices. If required to drive to carry out the duties of this position: current driver’s license and automobile insurance as required by Tennessee State Law.
