Under the mentoring and teaching of the VP of Clinical Operations, the Team Manager is responsible for upholding the role of learner, teacher, overseeing resource allocation, carrying out set standards, developing and coaching staff, and ultra-high accountability for creating a meaningful and powerful culture. The Team Manager is also responsible for monitoring and providing direct and daily supervision to the Clinical team and activities of daily visits and services to the patients and families. Build partnerships, working collaboratively with cross-function teams to provide oversight and support to meet the standards set forth. This position builds loyalty to Treasure Coast Hospice’s Mission, Vision and Values through set standards, techniques and collaborative role modeling.

Qualifications

 State of Florida RN License required.

 Must have three (3) years of customer service and five (5) years of managerial experience, preferred.

 MS Office Suite preferred. Netsmart EMR preferred.

 Ability to make sound decisions and “see the whole picture”.

 Ability to lead and teach an interdisciplinary team.

 Must be able to implement and execute, coach and mentor the set Standards of the organization.

 Strategic thinking, planning skills and the ability to interpret regulations and laws and disseminate information are necessary.

 Must have an entrepreneur drive to execute change and standards with integrity, energy, and enthusiasm.

 Excellent communication skills, collaboration, organizational ability, attention to detail, analytical skills, financial management, and the ability to lead a team.

 Attendance is a condition of employment.

 Valid Florida Driver’s License, reliable automobile and auto insurance.

Expectations

Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the Treasure Coast Hospice’s Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.

To meet or exceed identifiable standards for the position.

Key Result Areas

 Responsible for mastering, teaching, and adhering to and upholding agency-wide and clinical Standards and ensuring excellent customer service.

 Perform ride-alongs 60% of the time with field staff.

 Educates, mentors and coaches staff to ensure excellent customer service, high level staff performance and clinical excellence as measured by positive feedback from Family Satisfaction Survey, QI team performance, integrity of staff documentation.

 Responsible for the management and coordination of the day to day operations of patient care on the respective team. Outcomes measured by employee/client feedback, family outcomes/feedback, and employee performance levels. Works collaboratively with other leaders to accomplish this goal.

 Participate/provide leadership during the regular interdisciplinary team meetings, with oversight of the development, review and revision of the patient/family plan of care. Works with team to facilitate well organized productive and timely IDT meetings.

 Coordinate the effective use of DME and Pharmaceuticals.

 Effectively allocate resources by benchmarking and maintaining a budget based on Net Patient Revenue.

 Direct, monitor, and implement policies, procedures, and practices.

 Work collaboratively with peers, cross-functional team and internal departments.

 Responsible for the assignment of appropriate staff to provide care to patients and families, utilizing staffing standards to assure excellent end of life care.

 Accountable for the oversight of payroll.

 Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.

 Detect deviations from set Standards and hold team members accountable.

 Manage incidents and internal/external complaints in accordance with Treasure Coast Hospice policies.

 Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior.

 Lead, develop and motivate employees with confidence, energy, integrity and resources.

 Participate as an active supporter of the Treasure Coast Hospice compliance program including the emphasis to report, as well as complying with all Federal, State, and Local laws, as well as the organization’s compliance program.

 Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

OSHA Category

Position will be categorized as category III (involves no routine or potential exposure to blood, body fluids/tissues).

Working Conditions

 Typing or otherwise working primarily with fingers rather than with the whole hand as in handling.

 Sedentary – Exerting up to ten pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria met.

 Conduct ride-alongs with staff to teach and observe proper practices of patient care.

 Participate in management level meetings as needed.

 The work is required to have close visual acuity such as/preparing and analyzing data and figures; transcribing, viewing a computer terminal; extensive reading.

 Frequent lifting up to 10 pounds.
