The Opportunity
We're seeking a dynamic and adaptable Integrated Behavioral Health Specialist to join our growing team. In this key role, you will collaborate directly with primary care providers and other care team members to deliver integrated behavioral healthcare to patients across the age spectrum.

Your Impact
- Collaboration & Integration
- Work closely with primary care physicians, nurses, and other team members to deliver coordinated care.
- Participate in daily huddles, case consultations, and interdisciplinary team meetings.
- Facilitate warm handoffs and provide same-day consultations with patients in primary care.
- Offer behavioral health consultation and clinical support to primary care providers.
- Help develop integrated care plans that address both physical and mental health needs.
- Clinical Care
- Conduct brief behavioral health assessments, crisis interventions, and evidence-based interventions.
- Deliver short-term, solution-focused therapy and behavioral interventions in the primary care setting.
- Address a wide range of concerns including depression, anxiety, trauma, substance use, and adjustment issues.
- Provide care across the lifespan and support patients from diverse backgrounds.
- Program Development & Innovation
- Contribute to the growth and continuous improvement of integrated behavioral health services.
- Participate in quality improvement initiatives and data-informed outcome measurement.
- Support the implementation of new protocols, workflows, digital tools, and clinical innovations.
- Adapt clinical practices based on emerging best practices and evolving patient needs.
Who You Are
- Education & Licensure:
- Master’s degree in a behavioral health field (e.g., Social Work, Counseling, Marriage and Family Therapy, Psychology).
- Active state license in good standing, including one of the following:
- Licensed Associate Counselor (LAC)
- Licensed Professional Counselor (LPC)
- Licensed Master Social Worker (LMSW)
- Licensed Clinical Social Worker (LCSW)
- Associate Marriage and Family Therapist (AMFT)
- Licensed Marriage and Family Therapist (LMFT)
- Must maintain current license in accordance with state requirements and professional standards.
- Clinical Expertise & Approach:
- Integrated care, collaborative care models, or primary care behavioral health
- Proficiency in evidence-based practices such as CBT, ACT, or motivational interviewing
- Teamwork & Communication: Strong interpersonal skills and a team-based care mindset
- Adaptability: Comfort working in a fast-paced, evolving healthcare environment.
