Supervisor Discharge Planning bei Meritus Health

Monitors patient outcomes and facilitates quality clinical care within Brook Lane Health Services (BLHS) Hospital Programs (Inpatient) and Meritus Behavioral Health programs. Supports the treatment team in day-to-day psychiatric, medical, and psychosocial care coordination. Initiates and implements continuing care planning process during admission to meet patient post-discharge needs. Works in collaboration with other members of the treatment team and external agencies to facilitate efficient patient treatment along the continuum of care. Supervises Care Management Department.

Education – Minimum High School diploma required.  Bachelor’s or Master’s degree in Social Work, or Nursing or closely related field preferred. 

Experience – Minimum of 5 years of experience in care management, behavioral health, or human services field.

Licensure/Certification –  N/A

Knowledge/Skills/Abilities – Strong organization, time management and communication skills.  Knowledge of care coordination practice, strong knowledge of community resources in the areas of healthcare, mental health, substance abuse, housing, or other relevant providers.
