GENERAL SUMMARY:
Provide individual tracking and monitoring services both through face-to-face monitoring and with the Global Positioning System (GPS) to youth and families through a multi-agency community based collaborative service delivery system. Tracking of clients occurs throughout Milwaukee County at their homes, schools, jobs or community etc. Monitoring services occur 24/7 for all required and assigned youth to the program. Cross trained/dual role of Community Monitor and Youth Advocate to support both Level II and Intensive Monitoring Programs.
ESSENTIAL FUNCTIONS:
- Complete daily check-ins and document in appropriate system(s) per contract requirements.
- Develop and maintain healthy relationships with enrolled youth to encourage individual accountability and form trust.
- Routine tracks to ensure youth’s full compliance with the court’s orders and curfews.
- Curfew checks are required to be completed daily.
- Commit to working with any youth and/or family that is referred for the duration of their involvement in the program; acknowledge and emphasize the youth and family’s strengths, culture, and community.
- Monitor youth via phone, GPS monitoring, and/or through collateral contacts (other professionals, etc.).
- Enter data promptly in Agency documentation software and GPS tracking databases.
- Coordinate and complete assigned tracking/documentation per program expectations and requirements.
- Review case files to maintain familiarity with youth’s history.
- Support the family decision-making process to empower the youth and the family to avoid further law violations or continued involvement in the Juvenile Justice System.
- Prepare all documentation according to the program’s designated time frames.
- Present required reports to contracting entities including court officials.
- Review case files to become familiar with youth history.
- Maintain all required trainings and certifications.
- Sustain timely responses to all inquiries by youth, families, Service Providers and Milwaukee County Department of Youth & Family Services.
- Complete all required documentation in an accurate and timely manner.
- Adheres to St. Charles Youth & Family Services Policies and Procedures.
- Other duties as assigned.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- High School Diploma or equivalent required; Bachelor’s degree in Human Services, Criminal Justice, or related field preferred.
- One year of experience with programs that serve a delinquent or at-risk youth population.
- Knowledge base of adolescent and family issues and dynamics.
- Familiarity with database systems with ability to enter data frequently and timely.
- Knowledge of Milwaukee County (Department of Human Services, Children’s Court, Milwaukee Public Schools, and Milwaukee community based youth serving agencies.)
- Knowledge of human growth and development theories.
- Ability to prepare formal reports as requested
- Ability to present case in court if needed.
- Ability to document all interactions and activities in a timely manner.
- Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
- Ability to accurately perform record keeping and documentation entries.
- Excellent verbal and written communication skills.
- Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
- Ability to provide intervention programming and other programming as designed in collaboration with CYFS.
- Must possess a valid Wisconsin driver’s license and liability insurance and must meet agency’s driving requirements.
