Job Title:        Adult Day Center Program Assistant / CNA / Transportation Aide

Reports To:    Director, Adult Day Center 

Schedule:       Monday - Friday

Salary:           $15.68 - $19.68 an hour; Based on qualifications and experience

SUMMARY: Provides care and support to Adult Day Center participants and assists with daily activities and programs. Ensures safe and timely transportation for participants according to established routes and schedules.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

  • Assist with the planning and implementation of activities to include field trips, entertainment, special events and seasonal activities.
  • Administers direct personal care including assisting with personal hygiene, transfers, ambulation and prescribed treatment and activities.
  • Takes and records vital signs and monitors participant weight as prescribed by the center's policy and physician orders.
  • Assist in maintaining a safe, clean and positive environment for our participants.
  • Provides appropriate written information to prepare physician fax orders.
  • Assist in the set up and cleanup of activities; such as arts and crafts, cooking projects, games and physical activities.
  • Assist participants with personal hygiene; such as toileting and meal service, when necessary.
  • Assist with the transfer and ambulation of participants when necessary.
  • Documents care provided to participants and reports any significant observations of the participant's condition for the nurse and / or director to review.
  • Must be able to initiate CPR for participants in cardiac arrest and continue until assistance arrives. 
  • Possess the ability to give direction to staff, students and volunteers in the performance of their tasks when necessary.
  • Assist in the assessment of each participant and the development of the care plan. 
  • Attends to all other responsibilities as needed for the safety and well-being of the participants and the program. 
  • Reports all unsafe situations that put the participants, staff or the program in danger. 
  • Completes and reports all incident reports to the Director on the day the incident occurred. 
  • Performs all related duties as assigned by the Director. 
  • Serve as transportation coordinator for the Adult Day Center as needed. 
  • Perform and record the routine inspection of vehicles for preventive maintenance, including mileage, fuel usage, and other relevant data. 
  • Follow established schedules and routes. Comply with all federal and state regulations. 
  • Employee will assist passengers into and out of vehicles and secure passengers in their seats using seatbelts and/or other stabilizing devices. 

Qualifications: Ability to lead activities and involve and involve and motivate the elderly and our DDS population in a variety of therapeutic and stimulating activities. Ability to interact with younger and older individuals that have a variety of needs. Must possess skills conducive to open communication and development of rapport with the participants and their family members.  Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the individuals served. 

 

Education / Certifications: Shall possess a current certification in CPR/First Aid training and have the ability to recognize symptoms that require medical attention and intervention. Shall have completed and passed the appropriate certification as a Certified Nurse Aide. Must possess a current Driver's License. 

 

Physical and Mental Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The noise level at the center is a mixture of quiet and noisy depending on the participant's activities. Reasonable accommodation may be made to enable individuals with disabilities to perform essential functions.

 

Body Positions: While performing the duties of this job, the employee is regularly required to sit, stand, walk, bend, stoop, squat, twist and reach to administer assistance to participants. Employees stand and walk with participants at the center. Employees may sit with participants during meals and activities.

 

Hand Use: Uses hands to administer treatments, take and record vital signs, perform cleaning or cooking activities, and to assist participants with personal care. Must legibly record participant information and care given.

 

Strength: Employee assists with the transferring of participants from wheel chair to chair or toilet. Employees lift and carry tables and chairs for activity set up and clean up. Employee lifts and carries trays of dishes or glasses from table to kitchen for meal preparation and clean up. The employee must regularly lift and/or move up to 25 lbs., frequently lift and/or move up to 50 lbs., and occasionally help lift and/or move more than 100 lbs.

 

Pushing and Pulling: Employee may push participant in wheel chairs and may pull participants when assisting to stand up, get out of a chair, or enter a car or bus.

 

Sense of Touch: Employee uses sense of touch to take pulse and feel temperature of the participant's skin. Must be able to sense the amount of pressure applied when giving a massage or other nursing procedures.

 

Hearing: Must be able to communicate in person and over the phone. Must be able to hear participants and staff requesting help.

 

Vision: Must be able to visually assess participant's condition. Must be able to read and record appropriate documentation in participant's records.

 

Language Ability: Employee must be able to read to participants and write notes in participant records. Employees must tactfully communicate with participants, other staff members, volunteers, guests, and participant's family.
