About the Role:

New York Oncology Hematology (NYOH) is seeking a dedicated Outpatient Advanced Practice Provider (APP) to join our Hematology Oncology team in Albany, NY. In this clinic-based role, the APP will collaborate closely with Oncologists to manage the care of patients with complex blood disorders, providing high-quality, compassionate care in an outpatient setting. Responsibilities include clinical evaluation, disease monitoring, symptom management, patient and family education, and coordination of care. The APP will serve as a direct caregiver and clinical resource, helping to optimize patient outcomes and enhance the overall care experience.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Collaborates with oncologists in the outpatient management of all oncologic conditions.
  • Conducts comprehensive chart reviews, evaluates diagnostic results, and participates in the development and implementation of individualized care plans.
  • Performs routine follow-up visits, supportive care, and symptom management for patients undergoing active treatment or surveillance.
  • Orders and interprets diagnostic tests, including labs and imaging, and initiates appropriate interventions under established protocols or in collaboration with supervising physicians.
  • Coordinates care with the multidisciplinary team, including nurses, social workers, pharmacists, nutritionists, and other specialists to ensure a seamless care experience.
  • Provides education and counseling to patients and families regarding diagnosis, treatment plans, medications, and survivorship care.
  • Promotes preventive care and wellness strategies, connecting patients with appropriate community resources when needed.
  • Maintains timely and accurate documentation of all patient encounters, including visit notes and charge capture.
  • Integrates current evidence-based research and clinical guidelines into daily practice.
  • Participates in quality improvement efforts, staff education, and team-based initiatives.
  • Maintains working knowledge of reimbursement and documentation standards in the outpatient oncology setting.
  • Participates in routine APP team meetings and ongoing professional development.
  • This is a Monday–Friday outpatient position with no inpatient responsibilities.

MINIMUM QUALIFICATIONS:

Nurse Practitioner

  • Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program. This position is entry-level and typically requires a minimum of 0-3 years post MSN experience, preferably in the chosen specialty. RN state licensure and current registration with the State Board of Nursing in practicing state. Current state licensure as a Nurse Practitioner. Current basic life support (BLS)/CPR certification required.

Physician Assistant

  • Successful completion of an accredited Physician Assistant program. Current NCCPA (National Commission on Certification of Physician Assistants) certification. This position is entry-level and typically requires 0-3 years of experience as a practicing Physician Assistant, preferably in the chosen specialty. Current state license as a Physician Assistant in practicing state. Current basic life support (BLS)/CPR certification required.

Salary Transparency:

Exact compensation may vary based on skills, experience, and location.

Base Salary Range: between $125,000 - $135,000

About the Practice and their Mission:

New York Oncology Hematology (NYOH) is a large multi-specialty group in upstate New York with six cancer centers around New York State's Capital Region. We have various opportunities, including employment at large comprehensive cancer centers, to small, quaint rural offices. As part of our practice, we provide subspecialty oncology, hematology, stem cell, neuro-oncology, GYN oncology, mobile PET, and Women's Imaging. We are a progressive group and one of the few non-academic sites accredited to perform CAR-T therapy in the United States. Although a private practice, we are the sole hematology/oncology care providers at Albany Medical Center, the largest tertiary care/ academic center in eastern upstate NY.
