Compensation:

• $60–$70 per hour
• Pay delivered weekly via direct deposit

What You’ll Be Doing

As our Social Worker, you’ll be a vital advocate for residents—blending clinical expertise with compassion to elevate quality of care:

• Conduct thorough psychosocial assessments, develop and implement care plans, and support discharge planning.
• Act as an empathetic liaison for residents and families, ensuring dignity, involvement, and clear communication around care decisions.
• Respond to on-call needs during weekends, managing urgent support or crisis situations as required.
• Collaborate with interdisciplinary teams—nurses, therapists, admission staff—to deliver integrated, person-centered care.
• Connect with community resources, coordinate external services, and streamline transitions between care settings.
• Maintain detailed documentation and compliance with all relevant regulatory and ethical standards.

Requirements

Qualifications:

• Active LSW license required
• Bachelor’s Degree in Social Work or closely related field
• Minimum 3 years of experience as a Social Worker in a nursing home or similar long-term care setting
