The ReConnect Early Interventions for Older Adults Program is designed to identify and reduce mental health symptoms in adults age 60 and older in Orange County. It integrates evidence-based practices into culturally diversified care management and life coaching services, consisting of comprehensive psychosocial assessments, mental health screening, and supportive counseling, linkages to resources, support groups, and behavioral activation.
Position Objective: To enhance the mental health status, quality of life, and ability to live independently of Orange County older adults aged 60 and older by providing clinical case management services, short term individual therapy, and group therapy services.
JOB FUNCTIONS (including but not limited to):
· Conduct initial in-home assessments to determine participant’s psychosocial status.
· Provide clinical case management, education, and assistance in the overall coordination of services.
· Develop and implement participant’s individualized care plan to address unmet needs.
· Assist participants and their families in accessing appropriate community resources.
· Deliver short term individual therapy sessions to participants.
· Facilitate therapeutic support groups to participants.
· Educate providers regarding older adult behavioral health services as needed.
· Offer leadership and supervision to assigned support volunteers including MSWI.
· Represent the program among targeted community stakeholders.
· Compile and report case load statistics in a concise, timely manner to produce a monthly report.
· Conduct and track program evaluation to attain outcomes in compliance with funding sources.
· Other duties as assigned by supervisor.
Reporting To: Behavioral Health & Wellness Dept. Clinical Manager
Requirements
QUALIFICATIONS:
· Must have a master’s in social work (MSW)
· Must be fluent/proficient in either Spanish, Korean, Mandarin, OR Farsi.
· Knowledge of Orange County community services and resources.
· Ability to work effectively with diverse populations and in team settings.
· Outstanding communication and organizational skills.
· Excellent analytical, problem solving and organizational skills.
· Ability to develop a sense of trust and rapport with program’s clients.
· Ability to work independently and multi-task to achieve desire goals managing time efficiently in a fast-paced environment.
· Ability to maintain objectivity and confidentiality in documenting and discussing caseload.
· Proficient in modern office practices including basic computer skills.
· Proof of eligibility to work permanently in the United States.
· A valid California driver’s license, proof of current vehicle insurance, reliable transportation.
· Submit and successfully complete a LiveScan fingerprinting background check.
