Purpose of Position:

Plan, coordinate and monitor on-going activities for QCC residents from group interaction to an individual approach according to each resident needs and abilities under the direction of a qualified occupational therapist. 

 

Assist the MDS Coordinator to ensure that all of the Minimum Data Set (MDS) assessments, Care Area Assessment (CAA) summaries and documentation are consistend and in compliance with State and Federal requirements

 

Uphold the organization’s vision, mission, and corporate values.  Demonstrate understanding of and compliance with organization’s policies, procedures, code of conduct and work rules.

 

Essential Functions:

  • Design programs to encourage socialization, provide entertainment, relaxation, and fulfillment, and improve daily living skills.
  • Plan and offer a variety of activities and outtings throught the day and evening 7 days a week. 
  • Leads an activities program that is culturally relevant and sensitive to the residents interests and uses feedback from residents and families to remain effective.
  • Offer instructions to other staff as to how activity is to happen when the activity occurs during off-hours.
  • Set and promote resident and family council meetings monthly.
  • Conduct interviews to collect information from residents and their family members using the appropriate assessment instrument for MDS. 
  • Complete Minimum Data Set and Care Area Assessment (CAA) summaries and submit to the appropriate authority.
  • Write and monitor nursing care plans for residents.
  • Ensure the completeness of assessments and care plans making sure they are complete on time and meet the standard of practice and clinical guidelines and educate staff about the MDS assessment process.
  • Monitor  the daily census of the facility, monitors skilled nursing documentation and maintain other documentation to ensure consistency and compliance with regulations.
  • Utilizes data accessed through CASPER to help the facility monitor its own performance and identify areas for improvement
  • Participate in committees as directed. 
  • Lead Care Conference meetings with interdisiplanary team and family/POA/Resident Representative. 
  • Perform nursing duties as needed on the unit. 
  • Perform other duties as assigned.

Licensure, Certification, Etc.

 

Alaska Nursing License, BLS, Valid Alaska Driver's License; Alaska foodworker card, Must start actively working toward the federally approved Activities Director certification course upon hire and maintain an active Activities Director certification once it is obtained.
