Sangre de Cristo Community Care is looking for a Part-Time Registered Nurse to join our dynamic hospice team serving patients in Westcliffe, CO. Responsibilities:

· Plans, coordinates, implements, and evaluates the nursing care of all patients.

· Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs

· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.

Requirements

· Must be at least 21 years of age.

· A valid non-restricted RN license in the state of Colorado is required.

· Must have a valid non-restricted driver’s license.

· Flexibility and availability to work a weekend schedule if needed.

· At least 1 year of home health or hospice experience preferred.
