JOB SUMMARY

Position Title: Resource Liaison

Program: RISE

Classification: Non-Exempt/Full-Time

 

The Resource Liaison works closely with community members facing immediate, short-term needs, including housing, food insecurity, childcare, and more. Employing a person-centered approach, the Resource Liaison provides dedicated support to individuals moving from crisis to stability on their journey to a thriving future. This position requires flexibility to support the success of various programs in RISE.

 

DUTIES/RESPONSIBILITIES:

Program Implementation

  • Provides immediate support to families facing short-term crisis.
  • Works on assigned areas of focus, such as Community Health, Crispus Attucks Community Center, and Office of Aging.
  • Submits referrals on participant behalf and provides advocacy and community resources.

Data Management 

  • Maintains a comprehensive database while keeping participant information confidential.
  • Efficiently tracks financial spending and participant progress.
  • Updates database with current community resources and services.

Coaching Practices

  • Approaches work with a participant centered mindset.
  • Utilizes various tools and strategies to support participants.

Collaboration

  • Take initiative to connect community members with local services and resources.
  • Collaborates with CAP programs and services.
  • Nurtures relationships with community partners and local services.

Innovation

  • Participates actively in meetings.
  • Contributes to program evaluation and development.
  • Identifies new best practices for continuous improvements.

*Performs other related duties as assigned.

EDUCATION/EXPERIENCE REQUIREMENTS

 

  • Associate’s degree or four-year degree with minimum of three years’ experience or High School Diploma or equivalent minimum of five years’ experience working in human or social services.
  • Bilingual in Spanish/English preferred.
