The Physical Therapist PRN position in Oklahoma City, OK, offers a dynamic opportunity to provide high-quality rehabilitative care on an as-needed basis within a health care services setting. The primary goal of this role is to assess, plan, and implement individualized treatment programs that enhance patients' physical function, mobility, and overall quality of life. This position requires collaboration with interdisciplinary teams to ensure comprehensive patient care and to facilitate optimal recovery outcomes. The Physical Therapist will also educate patients and their families on injury prevention, therapeutic exercises, and health maintenance strategies. Flexibility and adaptability are essential, as the role involves responding to fluctuating patient volumes and diverse clinical needs in a supportive and professional environment.

As a Physical Therapist, you will:

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. 

• Applies concepts of infection control/universal precautions in performing patient care

• Assure continuity of quality patient care delivered with appropriate documentation

• Provide effective communication to patient/family, team members and other health care professionals

• Promote agency philosophy and administrative policies

• Maintain and complete all documentation within company stated guidelines. 

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.

• Valid Physical Therapy license

• 1 year of home health experience preferred

• CPR certification

• Valid Driver’s license

• Valid Auto Insurance

• HomeCare HomeBase experience preferred

Physical Demands

• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Must be able to lift, carry, pull, or push up to 100 lbs..
