As a Field-Based Care Manager, you’ll be a key part of that mission, ensuring our clients are receiving high quality care and building strong relationships with both caregivers and their families.

What You’ll Do:
Client Care & Case Management:
Conduct in-home visits to assess client satisfaction and care quality
Continuously update care plans based on changing client needs
Build strong, trusting relationships with clients and their families to ensure the highest level of care
Caregiver Support & Field Training:
Introduce caregivers to clients and provide hands-on training to ensure success
Observe, coach, and guide caregivers during field visits to promote high standards of care
Support new hires by instilling best practices, creating an environment of learning, and mentoring them
Quality Assurance & Compliance:
Conduct regular quality checks and document field observations
Ensure compliance with company standards and policies to maintain care excellence
Collect client and caregiver feedback to drive continuous improvement
What We’re Looking For:
Experience in Senior Care or Healthcare Management: Ideally, experience in home care or a similar field (CNA, LPN, or RN preferred, but not required)
Excellent Care Planning & Client Assessment Skills: You should be skilled at managing care plans and ensuring high-quality service
Confident Communicator & Supportive Leader: You’ll need to build relationships with caregivers, clients, and families, so being a strong communicator and a team player is essential
Ability to Work Independently: As this is a field-based role, you must be self-motivated and able to manage your time efficiently
Valid Driver’s License & Reliable Transportation: Willingness to travel throughout the service area
Hands-on Care: Experience in caring for seniors (professional or personal) (CNA, LPN, or RN a plus) At times you will be assiting our clients with their ADLs
Compensation & Benefits:
Salary: $40,000–$50,000 per year (based on experience)
Performance-Based Bonuses: Monthly performance bonuses based on key metrics
