Role Overview
- Location: Albany, NY
- Part-time 2 day/week schedule (flexible- weekends also)
- Post-acute care inpatient setting
What You'll Do
- Deliver comprehensive primary care
- Manage acute and chronic conditions
- Develop personalized care plans
- Integrate and work closely with the IDT
- Partner and work collaboratively with TeamHealth Physician
What we are Looking for
- Experienced clinicians
- FNP, CNP, AGNP, APRN, or PA
- Active New York NP/PA license
- Active DEA
