Hours: 8:00 AM - 4:30 PM
Pay: $32.51 - 40.41 per hour depending upon years of experience
Job Summary:
Under the supervision of the Social Work Manager, the Patient Navigator Assists with the process of referral for treatment. Works closely with Social Work Manager to ensure linkages in the community. Educates patients on the importance of outpatient behavioral health care and primary care follow-up. Facilitates and tracks referrals to community providers, including warm handoffs and appointments kept. Acts as a liaison with community agencies. Provides brief follow-up intervention services to patients that have been discharged until connections in the community are made.
Job Description:
- Provides assessments and identification of individuals that demonstrate the need for supportive care
- Perform outreach activities that connect people to appropriate health care
- Outreach, advocacy, preventative care, and wellness promotion
- Assist people to get health and social/economic services they need
- Data collection and research that supports community planning
- Teamwork and patient-centered care
- Assists with the process of referral for treatment; Works closely with social worker to ensure linkages in the community
- Works collaboratively with the other disciplines within the hospital and outpatient programs to ensure proper communication, handoffs and service linkages
- Serve as a member of an interdisciplinary treatment team in determining and implementing a comprehensive plan of care, including identifying and reducing barriers to treatment
- Educates patients on the importance of outpatient behavioral health care and primary care follow-up
- Facilitates and tracks referrals to community providers, including warm handoffs and appointments kept
- Acts as a liaison with community and governmental agencies
- Contributes to departmental QA and PI initiatives
- Provides brief follow-up intervention services to patients that have been discharged until connections in the community are made. Reminds patients of scheduled intake or follow-up appointments, including any specific instructions from providers. Accompanies patient on visits to initial treatment appointments as needed
- Other duties as assigned
Job Requirements:
- Minimum 2 years recent Health experience in a professional setting, working with psychiatric population, preferably with experience in care coordination, psychiatric social work inpatient, psychiatric emergency department, outpatient mental health or community crisis and response team
- BA/BS in Human Services, Social Work, or a related field
- Knowledge of HIPAA and Privacy requirements
- Demonstrates knowledge of SDOH, Health Equity, and Cultural Awareness
- Excellent telephonic and interpersonal communication skills
- Demonstrates a capacity to work collaboratively in a team setting, with an ability to listen to others, participate actively, and take appropriate responsibilities
- Demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to high standards of excellence
