POSITION SUMMARY

The Certified Substance Use Counselor (CSUC) provides coordinated, client-centered case management, connecting clients with substance use disorders (SUD) with the resources and services needed to support their recovery.

Location: 100% Remote

Part-Time, 20 hours Mon-Fri 8:00am - 5:00pm.


 

ESSENTIAL JOB DUTIES AND FUNCTIONS

  • Conducts comprehensive evaluations of a client's substance use and overall mental and physical health. Assess their readiness for treatment and identify any co-occurring mental health conditions.
  • Formulates and implements personalized treatment plans with clients and their families, tailoring them to individual needs, history, and recovery goals.
  • Coordinates client care by arranging for and referring clients to appropriate services, such as medical care, support groups, or job placement services.
  • Collaborates with the client to set reasonable, client-driven goals and create a plan for accessing services.
  • Connects clients with appropriate internal and external services, such as medical care, mental health treatment, housing, and employment.
  • Tracks client progress, reevaluating the plan, and making adjustments as needed.
  • Maintains accurate, detailed, and confidential client records, including progress notes and reports.
  • Participates in crisis interventions during emergency situations to ensure client safety.
  • Works with clients to identify relapse risks and develop prevention strategies to help them maintain their recovery.
  • Conducts outreach programs in the community to educate the public on the signs of addiction and other problematic behaviors.
  • Leads individual, group, and family therapy sessions to help clients develop coping skills, manage triggers, and alter destructive behaviors. Provides supportive and psycho-educational therapy.
  • Complies with all legal requirements and company policies.
  • Performs all other duties as assigned.


MINIMUM QUALIFICATION REQUIREMENTS

Education/Training/Experience

  • Bachelor’s degree in a behavioral health field such as Social Work, Psychology or Behavioral Sciences
  • Must have current CSAC, CADC, LADC, SAP,  or equivalent
  • Two years of experience in substance abuse or a related human services field.

Skills Knowledge

  • Strong knowledge of community resources and social service systems in Hawaii and an ability to creatively identify solutions for patient needs.
  • Demonstrated ability to conduct comprehensive psychosocial needs assessments.
  • Exceptional organizational and time management skills and attention to detail for managing patient referrals and follow-ups.
  • Proficiency in using electronic health records (EHR) systems and telehealth platforms, video software, and other digital health tools.
  • Excellent verbal and written communication, interpersonal, and advocacy skills.
  • Strong commitment to advocating for patients' rights and ensuring equitable access to resources.
  • Strong commitment to patient-centered care, cultural humility, and reducing health disparities.
  • A dedicated team player who communicates clearly and respectfully with all members of the care team (physicians, nurses, coordinators).
  • Understanding of HIPAA regulations and patient confidentiality.
