POSITION SUMMARY:
The Community Services Case Management Coordinator provides supervision to direct care staff and ensures that the Covenant House Mission and Principles are carried out through service delivery to youth. The Community Services Case Management Coordinator works closely with the Community Services Outreach Coordinator and the Director of Program Services to manage relationships with community partners and support the oversight of the Community Services Outreach Department programs: Rights of Passage Apartment Living (ROPAL), Community Services Center (CSC) and Drop-in/Resource Center programs.
ESSENTIAL FUNCTIONS: Covenant House Florida reserves the right to unilaterally modify, remove, or add to any of the following:
Develop, manage, supervise, and implement aspects of Community Services Department Programs (ROPAL, CSC and Drop-in/Resource Center). Utilize excellent interpersonal skills and strong written and verbal communication in order to effectively manage collaborative relationships with community partners.
Provide professional and administrative supervision and support to a team of case managers providing community services (ROPAL, CSC and Drop-in/Resource Center). These services must be rendered in a culturally competent manner according to CHF Mission, Principles, and Core Values.
Implement policies and procedures in relation to meeting youth’s immediate needs, triage/diversion services, long-term case management services, and incentive programs for the Community Services Department programs. Provide support and feedback to the Director of Program Services in the development of program changes with regards to evidenced based practices and community needs.
Oversee and ensure compliance in documentation into ETO, HMIS and other databases as indicated. Ensuring that data is accurate, concise and in a timely manner in accordance with agency policies, procedures, licensing, and grant requirements.
Ensure adherence to programmatic standards of documentation for ROPAL, CSC and Drop-in/Resource Center Programs as it relates to client files to ensure that all client files meet contractual obligations, and agency policies. Implement and facilitate a youth-led Youth Board meetings
Attend assigned meetings and trainings to maintain a knowledge of current best practices and procedures (i.e., youth-centered practices, harm reduction and supportive housing). Administer a comprehensive and collaborative system of care within Community Services Department which ensures the implementation of best practices in service delivery and case management, to include assessments, planning, implementation, evaluation, and transition/discharge processes.
Responsible for collecting, reviewing and preparing monthly program reports and participating in quarterly, annual, and program renewal reports as needed. Ensures accurate documentation and reporting of all services.
Facilitate and upkeep community partner relationships including agencies that provide support services to CHF Orlando youth.
Provide effective oversight of the upkeep of CHF equipment and property, to include but not limited to cleanliness and maintenance and inventory of agency vehicles, storage units, CSC Food Pantry, and Outreach storage areas. Work in conjunction with supervisory staff to ensure optimal physical plant maintenance, safety, and security. Provide follow-up and ensure compliance with all licensing standards. Complete work requests promptly as needed for repairs.
Assist with the placement, training, and ongoing supervision of CHF volunteers and interns.
Collaborate with Director of Program Services and Community Outreach Coordinator for opportunities for training and supportive supervision around trauma-informed care, harm reduction, relentless engagement, motivational interviewing, mental health first aid and other training priorities for engagement and working with youth experiencing homelessness and trafficking.
Drive company vehicles to escort youth to and from activities, hospital visits, school, and various other appointments, etc.
Ability to work evenings and weekends as needed in cases of crisis, program youth or staff issues, and special events. Provide administrative support, Administrator-on-Call (AOC) and Supervisor-on-Duty (SOD) as assigned.

JOB REQUIREMENTS:
EDUCATION: Bachelor’s Degree required (Masters strongly preferred) in a field related to Human Services.
EXPERIENCE: Three years of human services experience (two of which should be in a supervisory capacity), two years of experience working with an adolescent population preferred.
SKILLS: Knowledge of adolescent development, case management, and crisis intervention. Strong written and oral communication skills, excellent organizational and leadership skills to include scheduling, evaluation, monitoring of a budget, problem solving and conflict resolution skills. Strong interpersonal and public speaking skills. Must be a self-starter and team player with the ability to work with other staff members, clients, vendors, and the public.
SCHEDULE: Monday – Wednesday (8am – 6pm), Thursday (9am – 7pm). Flexibility is required. Schedules are subject to change to accommodate the needs of the youth and CHF.
TRANSPORTATION: Must have a valid Florida driver’s license and a good driving record and ability to drive CHF vehicles.
WORKING CONDITIONS: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee may work occasionally in outside weather conditions. The noise level in the work environment is usually moderate to loud. Must be able to lift up to 50 lbs. on occasion, bend, sit and stand for prolonged periods of time. Some weekend/holiday work will be required.
SUPERVISORY DUTIES The position serves as direct supervisor to ROPAL, CSC and Drop-in Resource Center Programs team members, and will be part of the Supervisor-On-Duty (SOD Administrator-on-Duty (AOC) rotation after completion of probationary period.
EQUIPMENT USED: Must be able to utilize a computer, tablet and cell phone as well as word processing programs, email, and web based programs. Must be able to communicate effectively both orally and in written form.
Due to licensing requirements, Covenant House Florida applicants will not be permitted to begin employment service until they have been cleared by a Florida and national criminal background search. All applicants for employment will be responsible for the cost associated with instituting background checks and will be eligible for reimbursement after 90 days of service.
