Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. Position Summary:
The Home Care Manager directs nursing care in accordance with approved policies, procedures, and professional standards of community nursing practices and coordinates all services provided by the Home Care department. Instrumental in managing the updated regulatory requirements to maintain home care program. Essential duties include hiring, training, and monitoring the home care team. Communicate and collaborate with the facility and program goals and ensure they are being met. Will assist in overseeing the financial responsibilities of the organization and assist with financial and revenue goals and measures while maintaining quality of care and staff professionalism. Responsibility entails oversight of ensuring goals to meet or exceed patient experience and outcomes. Will maintain and collaborate communication between leadership and staff to provide an optimal program and access to the program. Will assist in the program management oversight with the Home Care Director/Administrator including covering 24-hour call schedule.
Essential Functions:
- Collaborates with Director in the overall supervision of all home care staff- including registered nurses, home health aides, therapists, and support staff
- Assures that the professional standards of nursing practice are maintained
- Participates in the establishment of policies and procedures and assures that the policies are maintained
- Demonstrates understanding and expertise in all regulatory bodies and laws that pertain to survey readiness up to and including accreditation of the program and or future programs are met and or exceeded
- Participates in the selection of personnel for the agency
- Assists with PI and other applicable quality programs. Leads, participates in, or organizes teams that develop, test, and implement performance improvement solutions
- Functions as a resource for solutions regarding clinical care for physicians and all other staff
- Able to delegate assignments to team members
- Demonstrates flexibility in response to unexpected work volume, emergencies, and staffing changes
- Handles emergencies and crises situations well
- Provides feedback to nursing and support staff concerning appropriate and inappropriate interventions
- Oversees all operational activities of the Home Care agency to the benefit of the patients and the organization
- Provides day-to-day supervision and management of personnel in areas of assignment
- Treats and handles all patient information as confidential – addressing identified issues with staff members
- Assumes the responsibility of solving problems with patients, family members, staff, physicians, visitors, and members from other departments when necessary
- Oversight of care planning for areas of assignment
- Prepares and monitors operating and capital budgets for areas of assignment within established timeframes
- Responsible for compliance with regulatory standards
- Maintains, tracks, and reports staff educational requirements, certifications, and competencies
- Assures automated time-keeping system information is up-to-date and accurate for employees within department and submits time sheets within the established time frames
- Follows up with customers on complaints within 72 hours, then identifying steps to correct problem, communicating with staff, implementing plan, and providing feedback to customer using established guidelines
- Maintains knowledge of job-related duties; infection prevention & control, OSHA blood borne pathogens, TB/respiratory protection plan, policies and procedures
- Participates in disaster preparedness meetings, drug/procedural information update meetings, customer service meetings, leadership meetings, nursing meetings, quality improvement meetings, cross-departmental meetings related to Infusion therapy including pharmacy, prior authorization, billing, nursing, and others as assigned
- Supports organizational and departmental goals and seeks ways to meet or exceed goals. Leads and/or participates with the team to achieve targeted results
- Responsible for monitoring the compliance of all charging and the supporting documentation to ensure proper billing as appropriate in the department. Audits as needed
- Identifies opportunities for operational improvements and participates in necessary data collection and evaluation. Initiates changes per findings
- Performs additional duties and responsibilities as deemed necessary

Position Requirements:
- Required Illinois Nursing License
- Required is BLS/CPR certification within 3 months of hire
- Required 3 years of supervisory role in nursing
- Required 5 years’ experience in nursing practice
- Required 3 years’ experience in home care nursing related setting
- Required Bachelors Level Degree
- Preferred Master Level Degree
- Strong and effective communication and presentation skill sets
- Excellent skill set with understanding and preparation with strategic initiatives
- Ability to execute and excel at change management skill sets
- Leadership quality and understanding of emotional intelligence
- Identifies critical factors to analyze outcomes demonstrating effective and decisive decision making

Pay Range:
Pay ranges from $69,903k to $126,620k (rate of pay is based on applicable years of experience)

Hours: Monday- Friday with weekend availability
