JOB SUMMARY
The Registered Nurse is responsible for delivering high-quality nursing care to patients in their homes. This includes assessing patient and family needs, implementing the physician’s plan of care, and providing skilled nursing services within the scope of practice authorized by the State of Florida. The Registered Nurse ensures compliance with agency policies, state and local regulations, and accrediting body requirements while overseeing services provided by Licensed Practical Nurses, Home Health Aides, and Companion Homemakers assigned to the case. Reports to the Clinical Manager/ADON. Otherwise reports to the Director of Nursing or Administrator.
MINIMUM SKILLS AND QUALIFICATIONS
Graduate of an accredited school of nursing
Licensed Registered Nurse in good standing with the Florida Board of Nursing
At least 1 year of nursing experience; home care experience preferred
Evidence of satisfactory Level 2 Background Screening, OIG, and Sex Offender Registry clearance
Must be physically able to lift, push, or pull up to 50 pounds and perform described duties
Strong clinical assessment, documentation, and organizational skills
Adequate vision and hearing to respond to patient and agency needs
Fluent in both Spanish and English (speaking, reading, and writing required)
SPECIAL REQUIREMENTS
Demonstrates the highest level of integrity and professionalism at all times
Strong leadership, problem-solving, and supervisory skills
Excellent interpersonal and communication skills with patients, families, physicians, and staff
Knowledge of infection control, disease management, and patient safety practices
Ability to complete assessments within 48 hours of start of care
Must have PPD completed prior to patient visits
RESPONSIBILITIES AND DUTIES
Maintain accurate and up-to-date clinical records in compliance with agency policies
Implement and update the plan of care, including physician orders and amendments
Provide skilled nursing services, treatments, and procedures requiring specialized training
Observe and document patient symptoms, responses to treatment, and changes in condition, reporting to the Clinical Manager and physician as needed
Initiate preventive and rehabilitative nursing procedures appropriate to patient care and safety
Prepare and oversee care plans for home health aides
Counsel and educate patients and families on disease management and care strategies
Submit required documentation in a timely manner (nursing notes within 24 hours of shift completion)
Participate in continuing education, in-service training, and professional development as required
Reconcile medications at each visit and report abnormal findings within 24 hours
Demonstrate competency in wound care, wound vacs, IV therapy, and other specialized nursing skills (training available as needed)
Maintain strict confidentiality and compliance with HIPAA
ENVIRONMENTAL ELEMENTS
Work is performed in patients’ homes, which may vary from clean and well-maintained to cluttered or less sanitary environments. Conditions may also vary in temperature and comfort.
