UCFS is looking for candidates who are passionate about making a difference in the lives of others! We are currently seeking a full--time (40hrs) MDFT Therapist Assistant to provide case management & collaborative support services to adolescents who have behavioral/mental health challenges and/or who are abusing substances, and their parents. Services are provided to adolescents, caregivers, & family members.


Clients will be seen in Windham County.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assess clients’ needs in following domains: School, legal, economics, medical, social, work
  • Meet weekly w/MDFT therapist assigned to family to discuss tasks & progress for each case.
  • Provide weekly face to face direct service hours to client/family to teach advocacy & collaboration skills; partner w/family members to meet goals/objectives.
  • Provide a broad range of supports such as, job coaching, social skills, substance abuse recovery supports, training, mentoring, behavioral management skills and recreational activities as necessary
  • Set goals with each family and complete service agreement in agreed upon timeframe
  • Provide assistance to client in crisis situations
  • Remove barriers to MDFT treatment by addressing basic needs challenges.
  • Participate in case reviews and supervision; attend all regularly scheduled team meetings and training; maintain familiarity with community resources and collaborate with other agencies such as DCF and other community-based supports that are involved with the family
  • Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations and termination summaries
  • Minimum of 60% travel may be required.

Requirements

  • Bachelors Degree with one to three years prior experience or training in social services or Associates Degree in relevant field with three years of experience or training in social services
  • Experience working with children and families in recovery preferred or children and families with significant behavioral health needs.
  • Ability to provide a strength based, client centered approach to the delivery of service

OTHER SKILLS AND ABILITIES

  • Bilingual skills - Spanish/English preferred
  • Proficient with Microsoft Suite, Internet and e-mail
  • Valid driver's license and state mandated levels of auto liability insurance required
