Job Summary:
The Inpatient Case Manager is responsible for coordinating and managing the care of patients admitted to hospitals and skilled nursing facilities. This role ensures continuity of care by collaborating with providers, hospital staff, and patients to support safe transitions of care, optimize clinical outcomes, and reduce avoidable hospitalizations.

Key Responsibilities:

  • Coordinate and participate in daily patient rounds with hospital and nursing home staff.
  • Monitor inpatient admissions, discharges, and readmissions for assigned patients.
  • Collaborate with physicians, advanced practice providers, nursing staff, and other care team members to develop individualized patient care plans.
  • Ensure appropriate utilization of services and resources while maintaining quality of care.
  • Communicate regularly with patients and families to provide education, updates, and support regarding care plans and transitions of care.
  • Identify barriers to discharge and work with the care team to resolve them.
  • Provide timely updates and documentation in the patient’s electronic health record (EHR).
  • Facilitate coordination of post-acute services such as home health, rehabilitation, or outpatient follow-up.
  • Serve as a liaison between the practice, hospital staff, and community resources.
  • Track and report case management outcomes, including length of stay, readmission rates, and patient satisfaction.
  • Adhere to regulatory requirements and company policies regarding patient confidentiality and care standards.


Requirements

 Qualifications:

  • Minimum 2 years of clinical nursing experience; case management experience preferred.
  • Strong knowledge of inpatient and post-acute care processes.
  • Excellent communication, collaboration, and critical thinking skills.
  • Ability to manage multiple patients and priorities effectively.
  • Familiarity with EHR systems and care coordination tools.

Work Schedule:

  • Full-time, typically Monday–Friday, with flexibility to attend inpatient rounds and urgent patient care needs.

Physical Requirements:

  • Ability to travel between hospital, nursing facility, and office locations as needed.
  • Ability to sit, stand, and walk for extended periods.
