Inreach Case Worker Full TimeAbout the role
The Salvation Army Australia’s Homelessness Stream provides high quality, person-centred services to people at risk, and or experiencing homelessness. We support all cohorts in case management, outreach support, financial assistance, connection and referral to other specialist services.

We are seeking a values-driven and dedicated Case Worker, reporting to the Operations Co-ordinator. You will work with individuals in crisis to develop and implement case plans that promote their best interests and to ensure their safety and appropriate development. This is a permanent full-time position located at The Beacon, Northbridge, Western Australia.
You will successfully
- Build effective relationships are established with clients that encourage self-determination and participation
- Provide ongoing assessment of the client’s issues, needs and circumstances from initial intake to exit
- Provide Ongoing client risk assessment and pro-active flagging of concerns to Operations Coordinator
- Develop individual case plans and collaborate with clients to include a range of interventions to address needs or goals
- Assist clients to access support services and establish community support networks
- Keep accurate up to date client records in accordance with TSA standards
You will have
- Minimum Diploma Level in Community Services or Mental Health and or related experience in the industry.
- A current First Aid
- A current Driver's licence
- A willingness to undergo a National Police check
