Job Purpose: The Medical Chart Coder ensures accurate coding of provider documentation to support compliance, proper reimbursement, and high-quality patient care. This role is responsible for reviewing clinical documentation, assigning codes, and providing education to providers while maintaining strict adherence to coding guidelines and regulations.

This is a non-exempt position reporting to the Director of VP Medical Operations.

What You Will Do:

  • Conduct internal assessments of coding accuracy, documentation practices, and compliance with coding policies and strategies.
  • Audit patient records and generate reports to support quality assurance.
  • Prepare scorecards summarizing coding review findings.
  • Provide education and feedback to clinicians on proper coding guidelines and documentation practices.
  • Create, update, and maintain confidential patient information in strict compliance with HIPAA guidelines.
  • Perform routine data entry into EMR, billing software, insurance portals, and related systems.
  • Support scheduling and outreach activities; track and provide patient-related data as needed for reporting.
  • Answer and return phone calls, documenting interactions as appropriate.
  • Assist in preparing departmental reports to track outreach and performance metrics.
  • Perform other duties and special projects as assigned.

What We Need From You:

  • High school diploma or equivalent required.
  • Certified Professional Coder (CPC) in good standing with AAPC.
  • Minimum of 5 years progressive professional coding experience, with deep knowledge of ICD-10, CPT code assignment, reimbursement systems, and payer-specific regulations.
  • Strong understanding of documentation, coding, and billing compliance at federal, state, and payer levels.
  • Excellent communication skills, both verbal and written, with professional writing ability.
  • Proficiency with electronic record-keeping systems, including EMR, billing software, spreadsheets, and insurance carrier portals.)

Your Work Schedule and Environment:

  • Standard schedule: Monday–Friday, 8:00 AM to 5:00 PM
  • Hybrid after 90 days.
