Case Manager, Supportive Housing


Under the supervision of the Program Manager, the Case Manager will provide case management services to Supportive Housing clients seeking treatment for substance use disorders and co-occurring mental health disorders in an effort to support and strengthen their capacity to engage in healthy practices and to maintain stable households.  Additionally, the Case Manager will operate as a member of a larger team in the planning and provision of program services.  Local travel is required.  


 PERFORMANCE STANDARDS:

3.) Be of good character and reputation; sufficient physical, mental, and emotional health to satisfactorily perform her or his job duties.  

4.) Actively participate in safety programs of the organization by identifying potential risks and promoting client safety as well as environmental safety.  

5.) Accomplish individual and team goals and objectives established by supervisor based on prior performance evaluation.  

6.) Comply with HIPAA guidelines regarding client rights and confidentiality.  

7.) Comply with documentation and training set forth by Human Resources.  

8.) Actively participate in CGE's clinical research efforts by ensuring accurate and timely reporting for all assessments and evaluations. 


 CASE MANAGEMENT:  

• Provide direct case management services to clients for services including but not limited to:  educational and vocational pursuits, legal issues, recovery support, life skills (e.g. resume writing, budgeting, etc.), and accessing housing, social services and other community resources.  

• Assist with the onboarding of eligible clients to retain services through the Supportive Housing Program and assist with locating affordable housing in the community.  

• Assist clients with completion of all housing related documentation and communication with providers.  

• Provide outreach services to clients in the community and in their homes. Actively engage clients weekly or as needed.   

• Provide transportation to clients and their children as needed. 

• With client participation, and in collaboration with Supportive Housing team and with other providers, ensure that the individualized client goal plans are being met.    

• Complete case management documentation in a clear, concise, accurate and timely manner.  

• Assist with the collection, tracking, and documentation of all applicable client information and activities.  

• Maintain ongoing relationships with various community agencies/providers, and acts as a liaison to human service agencies that meet identified needs of clients.  

• Participate in program, committee and staff meetings and training program(s) as indicated.  

• Provide support to ensure seamless transitions among CGE’s continuum of care as needed.  

• Provide proper crisis management/intervention as needed and notify supervisor as soon as practical.   

• Exercise independent judgment when observing and assessing a client’s current functioning.   

• Respond to clients in an attuned and effective manner, employing CGE’s protocols and procedures as appropriate to presenting situation.  

• Make appropriate judgments with respect to life threatening circumstances and take appropriate action.  

• Co-facilitate client groups as appropriate.  

• Identify appropriate resources in the community for linkage, presentations, and activities to best support the clients. 

• Participate in program evaluation and ongoing quality improvement efforts.  

• Prepare and deliver oral and written reports to staff and appropriate professional agencies as indicated.  


GENERAL DUTIES:  

• Build a network of support within the program and among clients that focuses on trust, well-being and social/community integration.  

• Continually participate in building and maintaining positive working relationships through effective communication, performance improvement and teamwork efforts.  

• Participate in CGE staff meetings and trainings as requested.  

• Maintain records as required by organizational policies and regulatory and accrediting agencies.   

• Work as a member of a team to establish and maintain a high level of care and respect for, and communication with clients.   

• Work in a consistently professional manner at all times, which includes, but is not limited to, treating all residents, staff, guests, and volunteers with dignity and respect.   

• Remain flexible and available for day and evening hours.  

• Perform other duties as requested.   

Requirements

QUALIFICATIONS:  

• Master’s degree in human services, social work, psychology, or another related field or, student working toward a Master’s degree, or bachelor’s degree with equivalent experience.  

• LCADC/CADC a plus.  

• Minimum of one (1) year of experience working with substance use and co-occurring disorders.  

• Must have strong housing experience and knowledge of social services/community resources for clients.   

• Current driver's license needed; will be required to meet safe driving requirements; with ability to comfortably drive and maintain various sized vehicles.  

• Bilingual (English/Spanish) speaking a plus.  

• Excellent organizational, interpersonal, and documentation skills with friendly and professional demeanor.  

• Ability to be flexible to the daily changing needs within the community and to handle obstacles with compassion and resolve.
